带线锚钉Krackow缝合法联合可调式跟腱靴治疗闭合性跟腱断裂的效果

Effectiveness of Krackow Suture Method with Thread Anchor Nail Combined with Adjustable Achilles Tendon Boot in the Treatment of Closed Achilles Tendon Rupture

目的:探讨带线锚钉Krackow缝合法联合可调式跟腱靴治疗闭合性跟腱断裂的效果。方法:选取2020年8月—2022年6月常州市武进中医医院收治的闭合性跟腱断裂患者30例为观察对象,根据随机数字表法分为观察组和对照组,各15例。观察组采用带线锚钉Krackow缝合法联合可调式跟腱靴治疗,对照组采用带线锚钉Krackow缝合法联合常规石膏固定治疗。比较两组住院时间、美国足踝外科学会踝-后足评分系统(AOFAS)评分、视觉模拟量表(VAS)评分、术后并发症。结果:两组术后随访时间均为6个月。末次随访时,观察组AOFAS评分高于对照组,差异有统计学意义(P=0.03);术后第4、12、24周,观察组视觉模拟评分低于对照组,差异有统计学意义(P<0.05);对照组出现术后切口感染1例,其余患者无明显并发症发生。结论:带线锚钉Krackow缝合法联合可调式跟腱靴治疗闭合性跟腱断裂,患者早期功能康复效果较好,疼痛程度轻,安全性高。

Objective:To investigate the effect of Krackow suture method with thread anchor nail combined with adjustable Achil⁃les tendon boot in the treatment of closed Achilles tendon rupture.Methods:Thirty patients with closed Achilles tendon rupture ad⁃mitted to Changzhou Wujin Hospital of Traditional Chinese Medicine from August 2020 to June 2022 were selected for observa⁃tion,and they were divided into observation group and control group according to randomized numerical table method,each with fifteen cases.The observation group was treated with Krackow suture method with thread anchor nail combined with adjustable Achilles tendon boot,and the control group was treated with Krackow suture method with thread anchor nail combined with con⁃ventional plaster immobilization.The hospitalization time,American Society of Foot and Ankle Surgeons Ankle-Hindfoot Scoring System(AOFAS)score,Visual Analog Scale(VAS)score,and postoperative complications were compared between the two groups.Results:The postoperative follow-up time was 6 months in both groups.At the last follow-up,the AOFAS score of the observation group was higher than that of the control group,and the difference was statistically significant(P=0.03);At postoperative weeks 4,12,and 24,the visual analog score of the observation group was lower than that of the control group,and the difference was statisti⁃cally significant(P<0.05);One case of postoperative incision infection occurred in the control group,and no significant complica⁃tions occurred in the remaining patients.Conclusion:In the treatment of closed Achilles tendon rupture with Krackow suture with thread anchors combined with adjustable Achilles tendon boot,the early functional rehabilitation of the patients is better,the pain is less and the safety is high.
