血清miR-205-5p、miR-219-5p诊断子宫内膜癌及预判淋巴结转移的价值分析

Analysis of the value of serum miR-205-5p and miR-219-5p in the diagnosis of endometrial cancer and prediction of lymph node metastasis

目的:探讨血清微小RNA-205-5p(miR-205-5p)、miR-219-5p诊断子宫内膜癌(Endometrial carcinoma,EC)及预判淋巴结转移的临床价值。方法:选取我院2019年2月到2022年2月收治的120例子宫内膜癌患者为观察组,另选取同期健康体检女性100例为对照组。采用实时荧光定量PCR(qRT-PCR)检测纳入对象血清miR-205-5p、miR-219-5p水平,比较两组血清miR-205-5p、miR-219-5p水平,同时分析血清miR-205-5p、miR-219-5p水平与EC患者病理特征的关系。采用受试者工作特征(ROC)曲线评估血清miR-205-5p和miR-219-5p水平对EC淋巴结转移的预测价值,多因素Logistic回归分析探讨子宫内膜癌淋巴结转移的相关因素。结果:观察组血清miR-205-5p水平高于对照组,miR-219-5p低于对照组(P<0.05);子宫内膜癌患者血清miR-205-5p、miR-219-5p水平与病理类型、FIGO分期、组织学分级、肌层浸润、淋巴结转移有关(P<0.05)。血清miR-205-5p、miR-219-5p诊断子宫内膜癌的AUC为0.732、0.746,预判淋巴结转移的AUC为0.753、0.787。多因素Logistics回归分析显示,血清miR-205-5p高表达(OR=2.44,95%CI:1.62~3.67)、miR-219-5p低表达(OR=4.89,95%CI:2.55~9.39)均为子宫内膜癌淋巴结转移的危险因素(P<0.05)。结论:血清miR-205-5p在子宫内膜癌患者血清中呈高表达,miR-219-5p呈低表达,二者均与患者肌层浸润、淋巴结转移有关。

Objective:To explore the clinical value of serum microRNA-205-5p(miR-205-5p)and miR-219-5p in the diagnosis of endometrial cancer(EC)and in predicting lymph node metastasis.Methods:A total of 120 cases with endometrial cancer who were treated in our hospital from February 2019 to February 2022 were selected as the observation group,and 100 healthy women who underwent physical examination during the same period were selected as the control group.Serum levels of miR-205-5p and miR-219-5p were detected by real-time quantitative PCR(qRT-PCR).The differences in serum miR-205-5p and miR-219-5p levels between the two groups were compared,and the relationship between serum miR-205-5p,miR-219-5p levels and pathological characteristics of EC patients was analyzed.ROC curve was used to evaluate the predictive value of serum miR-205-5p and miR-219-5p levels in EC lymph node metastasis,and multivariate logistic regression analysis was used to explore the related factors of endometrial cancer lymph node metastasis.Results:The serum level of miR-205-5p in the observation group was higher than that in the control group,while the level of miR-219-5p in the observation group was lower than that in the control group(P<0.05).Serum levels of miR-205-5p and miR-219-5p in patients with endometrial cancer were related to pathological type,FIGO stage,histological grade,myometrial invasion and lymph node metastasis(P<0.05).The AUCs of serum miR-205-5p and miR-219-5p for the diagnosis of endometrial cancer were 0.732 and 0.746,and the AUCs for predicting lymph node metastasis were 0.753 and 0.787.High expression of serum miR-205-5p(OR=2.44,95%CI:1.62-3.67)and the low expression of miR-219-5p(OR=4.89,95%CI:2.55-9.39)were both risk factors for lymph node metastasis of endometrial cancer(P<0.05).Conclusion:The high expression of serum miR-205-5p in the serum of endometrial cancer patients and the low expression of miR-219-5p were both related to myometrial invasion and lymph node metastasis.
