达格列净联合非布司他治疗2型糖尿病合并痛风的临床效果

Clinical effect of dapagliflozin combined with febuxostat in the treatment of type 2 diabetes mellitus complicated with gout

目的探讨达格列净联合非布司他治疗2型糖尿病合并痛风的临床效果。方法选取100例2型糖尿病合并痛风患者为研究对象,根据治疗方案的不同将其分为对照组和观察组,各50例。对照组给予常规治疗结合非布他司治疗,观察组在对照组基础上联合达格列净治疗。比较两组的治疗效果。结果治疗后,观察组的空腹血糖(FBG)、餐后2 h血糖(2 h PBG)水平及糖化血红蛋白(Hb A1c)、胰岛素抵抗指数(HOMA-IR)低于对照组,空腹胰岛素(FINS)、餐后2 h胰岛素(2 h INS)、空腹C肽(FCP)、餐后2 h C肽(2 h CP)水平均高于对照组(P<0.05)。治疗后,观察组的尿酸(UA)、肿瘤坏死因子-α(TNF-α)、可溶性细胞间黏附分子-1(sICAM-1)及髓过氧化物酶(MPO)水平均低于对照组(P<0.05)。观察组的治疗总有效率高于对照组(P<0.05)。结论达格列净联合非布司他治疗2型糖尿病合并痛风的效果较好,值得推广及应用。

Objective To investigate the clinical effect of dapagliflozin combined with febuxostat in the treatment of type 2 diabetes mellitus complicated with gout.Methods A total of 100 patients with type 2 diabetes mellitus complicated with gout were selected as the research objects.According to different treatment schemes,the patients were divided into control group and observation group,with 50 cases in each group.The control group was treated with conventional therapy combined with febuxostat,and the observation group was treated with dapagliflozin on the basis of the control group.The therapeutic effects of the two groups were compared.Results After treatment,the fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PBG)levels,glycosylated hemoglobin(HbA1c)and homeostasis model insulin resistance index(HOMA-IR)in the observation group were lower than those in the control group,the levels of fasting insulin(FINS),2 h fasting insulin(2 h INS),fasting C-peptide(FCP),and 2 h postprandial C-peptide(2 h CP)were higher than those in the control group(P<0.05).After treatment,the levels of uric acid(UA),tumor necrosis factor-α(TNF-α),soluble intercellular adhesion molecules-1(sICAM-1)and myeloperoxidase(MPO)in the observation group were lower than those in the control group(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).Conclusion Dapagliflozin combined with febuxostat in the treatment of type 2 diabetes mellitus complicated with gout has better effect,which is worthy of promotion and opplication.
