屈螺酮炔雌醇片联合宫腔镜手术治疗子宫内膜息肉患者的效果

Effects of Drospirenone and ethinylestradiol tablets combined with hysteroscopic surgery in treatment of patients with endometrial polyps

目的:观察屈螺酮炔雌醇片联合宫腔镜手术治疗子宫内膜息肉(EP)患者的效果。方法:选取2019年1月至2021年12月该院收治的104例EP患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各52例。对照组给予宫腔镜手术,观察组在对照组基础上给予屈螺酮炔雌醇片治疗。比较两组临床疗效,治疗前后月经量[月经失血图(PBAC)评分]、子宫内膜厚度、经期时间、性激素指标[黄体生成素(LH)、孕酮、雌二醇(E_(2))]水平,以及复发率。结果:观察组治疗总有效率为94.23%,高于对照组的80.77%,差异有统计学意义(P<0.05);治疗后,观察组PBAC评分和子宫内膜厚度均低于对照组,经期时间短于对照组,差异有统计学意义(P<0.05);观察组LH、孕酮、E_(2)水平均低于对照组,差异有统计学意义(P<0.05);观察组复发率为5.77%,低于对照组的23.08%,差异有统计学意义(P<0.05)。结论:屈螺酮炔雌醇片联合宫腔镜手术治疗EP患者,可提高临床疗效,改善月经情况,降低子宫内膜厚度、性激素指标水平及复发率,效果优于单纯宫腔镜手术治疗。

Objective:To observe effects of Drospirenone and ethinylestradiol tablets combined with hysteroscopic surgery in treatment of patients with endometrial polyps(EP).Methods:A prospective study was conducted on 104 EP patients admitted to the hospital from January 2019 to December 2021.They were divided into control group and observation group according to the random number table method,52 cases in each group.The control group was given hysteroscopic surgery,while the observation group was treated with Drospirenone and ethinylestradiol tablets on the basis of that of the control group.The clinical efficacy,the menstrual volume before and after the treatment[pictorial blood loss assessment chart(PBAC)score],the endometrial thickness,the menstrual time,the sex hormone index[luteinizing hormone(LH),progesterone,estradiol(E_(2))]levels,and the recurrence rate were compared between the two groups.Results:The total effective rate of treatment in the observation group was 94.23%,which was higher than 80.77%in the control group,and the difference was statistically significant(P<0.05).After the treatment,the PBAC score and the endometrial thickness of the observation group were lower than those of the control group,the menstrual period was shorter than that of the control group,and the differences were statistically significant(P<0.05).The levels of LH,progesterone and E_(2) in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the recurrence rate of the observation group was 5.77%,which was lower than 23.08%of the control group,and the difference was statistically significant(P<0.05).Conclusions:Drospirenone and ethinylestradiol tablets combined with hysteroscopic surgery in the treatment of the EP patients can improve the clinical efficacy,improve the menstruation conditions and reduce the endometrial thickness,the sex hormone index levels and the recurrence rate.Moreover,it is superior to single hysteroscopic surgery.
