二甲双胍联合炔雌醇环丙孕酮片治疗多囊卵巢综合征患者的效果

Effects of Metformin combined with Ethinyl estradiol cyproterone tablets in treatment of patients with polycystic ovary syndrome

目的:观察二甲双胍联合炔雌醇环丙孕酮片治疗多囊卵巢综合征(PCOS)患者的效果。方法:选取2019年7月至2020年12月该院收治的86例PCOS患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各43例。对照组给予炔雌醇环丙孕酮片治疗,观察组在对照组基础上联合二甲双胍治疗。比较两组临床疗效,治疗前后糖脂代谢指标[总胆固醇(TC)、空腹血糖(FBG)、三酰甘油(TG)]、胰岛素指标[胰岛素抵抗指数(HOMA-IR)、空腹胰岛素(FINS)]、性激素指标[黄体生成素(LH)、睾酮、雌二醇(E_(2))]水平,以及不良反应发生率和排卵率。结果:观察组治疗总有效率为95.35%,高于对照组的76.74%,差异有统计学意义(P<0.05);治疗后,观察组TC、FBG、TG水平均低于对照组,差异有统计学意义(P<0.05);观察组HOMA-IR、FINS水平均低于对照组,差异有统计学意义(P<0.05);观察组LH、睾酮水平均低于对照组,E_(2)水平高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05);观察组排卵率为74.42%,高于对照组的53.49%,差异有统计学意义(P<0.05)。结论:二甲双胍联合炔雌醇环丙孕酮片治疗PCOS患者可提高治疗总有效率和排卵率,改善性激素指标水平,以及降低糖脂代谢指标和胰岛素指标水平,效果优于单纯炔雌醇环丙孕酮片治疗。

Objective:To observe effects of Metformin combined with Ethinyl estradiol cyproterone tablets in treatment of patients with polycystic ovary syndrome(PCOS).Methods:A prospective study was conducted on 86 patients with PCOS admitted to the hospital from July 2019 to December 2020.They were divided into control group and observation group according to the random number table method,43 cases in each group.The control group was treated with Ethinyl estradiol cyproterone tablets,while the observation group was treated with Metformin on the basis of that of the control group.The clinical efficacy,the levels of glucose and lipid metabolism indexes[total cholesterol(TC),fasting blood glucose(FBG),triglyceride(TG)],insulin indexes[insulin resistance index(HOMA-IR),fasting insulin(FINS)],the sex hormone levels[luteinizing hormone(LH),testosterone,estradiol(E_(2))],the incidence of adverse reactions,and the ovulation rate were compared between the two groups.Results:The total effective rate of treatment in the observation group was 95.35%,which was higher than 76.74%in the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of TC,FBG and TG in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of HOMA-IR and FINS in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of LH and testosterone in the observation group were lower than those in the control group,the level of E_(2) was higher than that in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).The ovulation rate of the observation group was 74.42%,which was higher than 53.49%of the control group,and the difference was statistically significant(P<0.05).Conclusions:Metformin combined with Ethinylestradiol and cyproterone acetate tablets in the treatment of the PCOS patients can improve the total effective rate and ovulation rate,improve the levels of sex hormones,and reduce the levels of glucose and lipid metabolism indexes and insulin indexes.Moreover,it is superior to single Ethinyl estradiol cyproterone tablets treatment.
