脑卒中后吞咽障碍患者的皮内针埋针治疗效果及对吞咽功能、表面肌电图的影响

Effect of Intradermal Needle Embedding on Swallowing Function And Surface Electromyography in Patients With Dysphagia After Stroke

目的:探究脑卒中后吞咽障碍患者的皮内针埋针治疗效果及对吞咽功能、表面肌电图的影响。方法:选取该院2020年4月至2022年4月收治的60例脑卒中后吞咽障碍患者,使用抽签法随机分组,对照组(n=30)予以神经肌肉电刺激和吞咽功能训练治疗,观察组(n=30)在对照组的基础上予以皮内针埋针治疗,观察两组患者临床疗效、表面肌电信号(sEMG)、吞咽功能、吞咽患者生活质量量表(SWAL-QOL)、美国国立卫生研究院卒中量表(NIHSS)评分、不良反应发生率。结果:观察组总有效率优于对照组(P<0.05)。治疗后,观察组最大波幅值高于对照组,吞咽时程低于对照组(P<0.05)。治疗后,观察组口腔期、咽期、误咽程度及总评分均高于对照组(P<0.05)。治疗后,观察组生活质量、NIHSS评分均优于对照组(P<0.05)。观察组不良反应发生率低于对照组(P<0.05)。结论:予以脑卒中后吞咽障碍患者皮内针埋针治疗,有利于改善患者神经功能与吞咽功能,增强生活质量水平,临床疗效显著,值得临床推广。

Objective:To explore the therapeutic effect of intradermal needle embedding in patients with dysphagia after stroke and its influence on swallowing function and surface electromyography.Methods:Sixty patients with dysphagia after stroke admitted to the hospital from April 2020 April 2022 were randomly divided by lottery method.The control group(n=30)was given neuromuscular electrical stimulation and swallowing func‐tion training,and the observation group(n=30)was also given intradermal acupuncture treatment.Clinical efficacy,surface electromyography(sEMG),swallowing function,SWAL-QOL,NIHSS score and incidence of reactions were observed in 2 groups.Results:The total effective rate of the observation group was better than that of the control group(P<0.05).After treatment,the maximum amplitude and swallowing duration in the ob‐servation group were better than those in the control group(P<0.05).After treatment,the oral stage,pharyngeal stage,misopharyngeal degree and to‐tal score of observation group were higher than those of control group(P<0.05).After treatment,SWAL-QOL score and NIHSS score in the observa‐tion group were lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was 3.33%lower than that in the control group(P<0.05).Conclusion:Intradermal acupuncture treatment for patients with dysphagia after stroke is beneficial to im‐prove the neurological function and dysphagia function,and enhance the quality of life.The clinical effect is significant,worthy of clinical promotion.
