老年鼻咽癌EB病毒感染及其对癌组织MUC16和AKR1B10表达的影响

EB virus infection in elderly patients with nasopharyngeal carcinoma and its influence on expressions of cancer tissue MUC16 and AKR1B10

目的探究老年鼻咽癌EB病毒(EBV)感染及其对癌组织黏蛋白16(MUC16)和醛酮还原酶家族1成员B10(AKR1B10)表达的影响。方法选择中南大学湘雅医学院附属海口医院2019年9月-2021年9月收治的老年鼻咽癌患者89例为研究组,另随机选择同期因慢性鼻咽炎进行活检的老年患者70例为对照组。采用荧光定量聚合酶链反应检测两组患者全血EB病毒核酸载量,采用免疫组织化学法检测两组患者组织标本中MUC16、AKR1B10蛋白表达情况,比较两组检测结果差异和研究组中不同EBV载量患者检测结果差异。结果研究组与对照组EBV感染率分别为49.44%和5.71%,研究组EBV阳性患者中EBV核酸载量主要集中于500~4999 copies/ml(65.91%);研究组组织标本中MUC16高表达率低于对照组,而AKR1B10高表达率高于对照组(P<0.05);研究组患者组织标本中AKR1B10高表达率为92.13%,高于对照组的12.86%(P<0.05);研究组EBV阳性患者组织标本MUC16表达率低于EBV阴性患者,而AKR1B10高表达率高于EBV阴性患者(P<0.05)。结论MUC16表达下调和AKR1B10表达上调可能与老年鼻咽癌的发生有关,且EBV感染可能导致老年鼻咽癌患者MUC16、AKR1B10表达水平进一步下调或上调。

OBJECTIVE To explore the Epstein-Barr virus(EBV)infection in elderly patients with nasopharyngeal carcinoma and observe its influence on expressions of cancer tissues mucin16(MUC16)and aldo-keto reductase family1member B10(AKR1B10).METHODS Totally 89elderly patients with nasopharyngeal carcinoma who were treated in Haikou Affiliated Hospital of Central South University Xiangya School of Medicine from Sep 2019 to Sep 2021were assigned as the study group,meanwhile,70elderly patients who underwent biopsy due to chronic nasopharyngitis were chosen as the control group.The whole blood EB virus nucleic acid load was detected for the two groups of patients by fluorescent quantitative polymerase chain reaction.The expression levels of MUC16 and AKR1B10in cancer tissue samples were detected by immunohistochemistry.The detection results were compared between the two groups and among the patients with different EBV loads in the study group.RESULTS The incidence rate of EBV infection was 49.44%in the study group,5.71%in the control group.The EBV nucleic acid load of the EBV-positive patients of the study group mainly ranged between 500and 4999copies/ml(65.91%).The high expression rate of tissue sample MUC16of the study group was lower than that of the control group,while the high expression rate of AKR1B10of the study group was higher than that of the control group(P<0.05).The high expression rate of tissue sample AKR1B10of the study group was 92.13%,higher than 12.86%of the control group(P<0.05).In the study group,the expression rate of tissue sample MUC16of the EBV-positive patients was lower than that of the EBV-negative patients,while the high expression rate of AKR1B10of the EBV-positive patients was higher than that of the EBV-negative patients(P<0.05).CONCLUSIONThe downregulated expression of MUC16and upregulated expression of AKR1B10may be associated with the occurrence of nasopharyngeal carcinoma in the elderly patients,and the EBV infection may further result in the upregulation or downregulation of expression levels of MUC16and AKR1B10in the elderly patients with nasopharyngeal carcinoma.
