AQP4、S100β基因在感染性脑积水中的表达及临床意义

摘要 目的分析水通道蛋白4(AQP4)、中枢神经特异蛋白(S100β)基因在感染性脑积水中的表达及临床意义。方法选取2019年9月-2021年3月于青岛滨海学院附属医院和山东第一医科大学附属人民医院接受治疗的脑积水患者120例为研究对象,按照合并细菌感染情况分为感染组49例、未感染组71例。荧光定量聚合酶链式反应(PCR)法检测AQP4表达,酶联免疫吸附法检测S100β表达。Pearson相关性对AQP4、S100β相关性进行分析,受试者工作特征(ROC)曲线分析AQP4、S100β对感染性脑积水的预测价值。结果与未感染组比较,感染组患者S100β、AQP4表达较高、急性生理与慢性健康Ⅱ评分(APACHEⅡ)、视觉模拟评分(VAS)较高(P<0.05),在感染性脑积水疾病中,APACHEⅡ正相关于VAS,AQP4正相关于S100β(P<0.05)。与轻症感染性脑积水患者比较,重症感染性脑积水患者S100β、AQP4表达较高(P<0.05),ROC曲线显示,AQP4、S100β联合检测对感染性脑积水的预测价值优于单一检测(P<0.05)。结论AQP4、S100β在感染性脑积水疾病中表达异常升高,可作为感染性脑积水的基因检测指标。 OBJECTIVE To analyze the expressions of aquaporin 4(AQP4)and central nervous system specific protein(S100β)genes in infectious hydrocephalus and observe the clinical significance.METHODS A total of 120 patients with hydrocephalus who were treated in the Affiliated Hospital of Qingdao Binhai University and People's Hospital Affiliated to Shandong First Medical University from Sep 2019 to Mar 2021 were recruited as the research subjects and divided into the infection group with 49 cases and the no infection group with 71 cases according to the status of bacterial infection.The expression of AQP4 was detected by fluorescent quantitative polymerase chain reaction(PCR),the expression of S1ooβwas detected by enzyme-linked immunosorbent assay.Pearson correlation analysis was performed for the correlation between AQP4 and S1ooβ,and the values of AQP4 and S1oop in prediction of infectious hydrocephalus were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS The expression levels of AQP4 and S10oβof the infection group were significantly higher than those of the no infection group;the scores of acute physiology and chronic health evaluation(APACHEII)and visual analogue scale(VAS)of the infection group were significantly higher than those of the no infection group(P<0.05).Among the patients with infectious hydrocephalus,APACHEI was positively correlated with VAS,and AQP4 was positively correlated with S100β(P<0.05).The expression levels of S100βand AQP4 of the patients with severe infectious hydrocephalus were significantly higher than those of the patients with mild infectious hydrocephalus(P<0.05).ROC curve analysis showed that the value of the joint detection of AQP4 and S100βwas significantly higher than that of the single detection in prediction of infectious hydrocephalus(P<o.05).CONCLUSION The patients with infectious hydrocephalus show abnormal rise of expressions of AQP4 and S10oβ,which can be used as genetic test indexes for infectious hydrocephalus.
