基于中断时间序列的白内障日间手术住院费用分析

An Interrupted Time Series-based Study of Hospitalization Expenses for Cataract Day Surgery

目的研究某院实施白内障日间手术对住院费用的影响。方法调取2015年1月1日-2021年12月31日某院实施白内障日间手术患者信息,采用中断时间序列分析方法对日间手术前后的住院医疗费用以及费用结构的水平变化和趋势变化进行分析。结果住院费用、综合医疗服务费、诊断费、药品费、医用材料费、其他费用较日间手术开展前都有所下降,医用材料费所占比重较高;中断时间序列分析结果显示,综合医疗服务费较日间手术开展之前每月下降2.95元,医用材料费每月下降29.41元,治疗费、药品费和其他费用在日间手术开展后瞬时下降1032.76元、499.94元和7.9元,统计显示综合医疗服务费、医用材料费、药品费在日间手术实施后都呈现下降趋势。结论白内障日间手术的开展,使得除治疗费外的其他各类费用均有所下降,有利于降低患者的住院医疗费用,减轻患者的疾病负担,促进费用结构的优化。

Objectives This study aims to study the impact of cataract day surgery on hospitalization costs in a hospital.Methods The information on cataract day surgery patients in a hospital from January 1,2015 to December 31,2021 was retrieved.Interrupted time series analysis was used to analyze the level changes and trend changes of inpatient medical costs and cost structure before and after the implementation of cataract day surgery in a hospital.Results Inpatient costs,comprehensive medical service fees,diagnostic fees,drug fees,medical material fees,and other costs all decreased compared with those before day surgery was performed.Medical material fees accounted for a higher proportion.The results of the interrupted time series analysis showed that comprehensive medical service fees decreased by 2.95 yuan per month compared with those before the day surgery was performed.Medical material fees decreased by 29.41 yuan per month.Treatment fees,drug fees,and other fees instantaneously decreased by 1032.76 yuan,499.94 yuan,and 7.9 yuan per month after the day surgery was performed.The comprehensive medical service fee,medical material fee,and drug fee all showed a decreasing trend after the implementation of day surgery.Conclusions The implementation of day surgery for cataracts has led to a decrease in all kinds of costs except treatment costs,which is conducive to reducing patients'inpatient medical costs,alleviating patients'disease burden,and promoting the optimization of the cost structure.
