北京市某三甲医院骨科超长住院日影响因素分析

Analysis of Influencing Factors of Ultra-long Hospitalization Days in Orthopedics Department of a Tertiary Hospital in Beijing

目的了解骨科患者的住院日水平,探讨影响超长住院日的因素。方法收集2021年1月1日-2021年12月31日医院骨科出院患者的住院病案首页资料,描述患者的基本情况,采用单因素和多因素Logistic回归模型分析超长住院日的影响因素。结果共纳入34235例骨科住院患者,其中超长住院患者175例,平均住院日为43.73±22.52天;多因素Logistic回归分析结果显示患者来自外省市(OR=3.82,95%CI 2.55~5.74)、付费方式为新型农村合作医疗(OR=2.12,95%CI 1.08~4.14)或其他(OR=3.76,95%CI 2.37~5.98)、住院期间转科(OR=13.78,95%CI 9.11~20.84)是患者超长住院的危险因素;年龄≥65岁(OR=0.39,95%CI 0.18~0.82)、女性(OR=0.60,95%CI 0.43~0.83)是超长住院的保护因素。结论年龄、性别、是否来自外省市、付费方式、是否转科是骨科患者超长住院日的主要影响因素。

Objectives This study aims to investigate the level of hospitalization days for orthopedic patients and explore the factors influencing ultra-long hospitalization days.Methods Information on medical record front pages of discharged patients from January 1,2021 to December 31,2021 was collected to describe the basic conditions of the patients.The factors influencing the extra-long hospital stay were analyzed using univariate and multivariate logistic regression models.Results A total of 34,235 study subjects were included in this study.Among them,175 patients were hospitalized for a long time,and the average length of hospitalization was 43.73±22.52 days.The results of the multivariate logistic regression analysis showed that other province and cities where the patients were from(OR=3.82,95%CI 2.55-5.74),the payment method of new rural cooperative medical care(OR=2.12,95%CI 1.08-4.14)and other(OR=3.76,95%CI 2.37-5.98),and transferring departments(OR=13.78,95%CI 9.11-20.84)were risk factors for ultra-long hospital days.Age≥65 years old(OR=0.39,95%CI 0.18-0.82)and female(OR=0.60,95%CI 0.43-0.83)were protective factors for excessively long hospitalization.Conclusions Age,gender,whether they are from other provinces or cities,payment methods,and whether they have transferred to other departments are the main factors affecting the long hospital stay of orthopedic patients.
