保乳联合前哨淋巴结活检术治疗早期乳腺癌患者的临床疗效评价

Clinical Effect Evaluation of Breast Conserving Combined with Sentinel Lymph Node Biopsy in Treatment of Early Breast Cancer

目的 探讨在早期乳腺癌患者的治疗中保乳联合前哨淋巴结活检术的应用价值。方法 选取2021年6月-2022年10月本院收治的80例早期乳腺癌患者,根据治疗手段将其分成对照组(n=40,乳腺癌改良根治术)与研究组(n=40,保乳联合前哨淋巴结活检术),对比临床指标转归情况、关节活动度、生活质量评分、乳房美容效果以及并发症发生率。结果 研究组手术时间、腋窝拔管时间以及住院时间均较短(P<0.05);研究组术中出血量、总引流量均较少(P<0.05);研究组肩关节活动度中外旋、内旋、外展、后伸及屈伸度均较大(P<0.05);研究组SF-36量表评分较高(P<0.05);研究组乳房美容优良率较高(P<0.05);研究组皮瓣坏死、皮下积液、上肢水肿较低(P<0.05)。结论 在早期乳腺癌患者治疗中采取保乳联合前哨淋巴结活检术可改善肩关节活动度,缩短治疗时间,乳房美容效果较好,患者生活质量显著改善,建议推广并临床广泛应用。

Objective To explore application value of breast conserving combined with sentinel lymph node biopsy in treatment of early breast cancer.Methods The paper chose 80 patients with early breast cancer admitted to our hospital from June 2021 to October 2022,and divided them into control group(n=40,with modified radical mastectomy for breast cancer)and study group(n=40,with breast conserving combined with sentinel lymph node biopsy)according to treatment methods.Clinical indicators,joint mobility,life quality scores,breast beauty effect and incidence of complications were compared.Results Study group had shorter surgical time,less intraoperative bleeding volume,total drainage volume,shorter axillary extubation time and length of stay(P<0.05).Range of motion of the shoulder joint,including external rotation,internal rotation,abduction,backward extension and flexion and extension,in study group was greater,(P<0.05).SF-36 scale scores in research group were relatively higher,(P<0.05).Research group had higher rate of excellent breast beauty(P<0.05).Study group had lower levels of flap necrosis,subcutaneous fluid accumulation and upper limb edema(P<0.05).Conclusion Breast conserving combined with sentinel lymph node biopsy can improve range of motion of shoulder joint,shorten treatment time in treatment of early breast cancer patients,with better breast beauty effect,can improve life quality of patients significantly.It is recommended to promote and apply it in clinical practice widely.
