温针灸联合电针对肛肠术后尿潴留患者排尿功能、尿动力学状态及预后的影响

Effects of warm acupuncture combined with electroacupuncture on urination function,urodynamic status and prognosis of patients with urinary retention after anorectal surgery

目的:探究温针灸联合电针对肛肠术后尿潴留患者排尿功能、尿动力学状态及预后的影响。方法:选取80例肛肠术后尿潴留患者,按照随机数字表法分为试验组和对照组,每组40例。对照组采用电针治疗,试验组在此基础上联合温针灸治疗,观察两组治疗后的疗效、生活质量和排尿功能及治疗前后尿动力学指标(膀胱初感容积、最大尿流率、最大逼尿肌压力)、中医症候的变化情况。结果:治疗后,试验组治疗总有效率高于对照组(P<0.05);试验组首次自行排尿时间、首次排尿后膀胱残留尿量均明显低于对照组,2 h内排尿总量明显高于对照组(P<0.05);试验组最大尿流率、最大逼尿肌压力高于对照组,膀胱初感容积低于对照组(P<0.05);试验组主症和次症积分均明显低于对照组(P<0.05);试验组生活质量各项得分均明显高于对照组(P<0.05)。结论:应用温针灸和电针联合治疗肛肠术后尿潴留患者,其疗效良好,能改善患者排尿功能和尿动力学状态,减少中医症候积分。

Objective:To explore the effects of warm acupuncture combined with electroacupuncture on urination function,urodynamic status and prognosis of patients with urinary retention after anorectal surgery.Methods:A total of 80 patients with urinary retention after anorectal surgery were enrolled.According to random number table method,they were divided into experimental group and control group,40 cases in each group.The control group was treated with electroacupuncture,while experimental group was additionally treated with warm acupuncture.The curative effect,quality of life and urination function after treatment,changes of urodynamic indexes(bladder tactile volume,maximum flow rate,maximum detrusor pressure)and TCM syndromes before and after treatment in the two groups were observed.Results:After treatment,total response rate of treatment in experimental group was higher than that in control group(P<0.05).The first self-urination time and bladder residual urine volume in experimental group were significantly lower than those in control group,while total urine output within 2 h was significantly more than that in control group(P<0.05).The maximum flow rate and maximum detrusor pressure in experimental group were significantly higher than those in control group,while the bladder tactile volume was smaller than that in control group(P<0.05).The scores of primary and secondary symptoms in experimental group were significantly lower than those in control group(P<0.05),and scores of quality of life were significantly higher than those in control group(P<0.05).Conclusion:The curative effect of warm acupuncture combined with electroacupuncture is good in patients with urinary retention after anorectal surgery,which can improve urination function and urodynamic status,reduce scores of TCM syndromes.
