伴侣参与认知访谈联合希望干预对复发性流产患者心理状态、应对方式的影响

Influence of partner's participation in cognitive interview combined with hope intervention on psychological status and coping styles in patients with recurrent spontaneous abortion

目的探讨分析伴侣参与访谈联合希望干预对复发性流产患者心理状态、应对方式的影响。方法选取2019年4月至2021年4月期间复旦大学附属妇产科医院收治的复发性流产患者123例,采用数字表法随机为观察组62例、对照组61例,对照组予以常规护理联合希望干预,观察组在对照组基础上采用伴侣参与认知访谈干预。2 w后,采用汉密顿焦虑量表(HAMA)与汉密顿抑郁量表(HAMD)评估两组干预前后的心理状态、采用医学应对问卷(MCMQ)评估两组心理应对方式、采用Herth希望量表(HHI)评估患者希望水平、采用生活质量调查表(SF-36)评估两组生活质量。结果干预前两组HAMA评分和HAMD评分比较差异无统计学意义(P>0.05),干预后两组上述两个量表评分均降低、且观察组两个量表评分均低于对照组(P<0.05);干预前两组应对方式各维度评分比较差异无统计学意义(P>0.05),干预后两组面对维度评分均升高、回避和屈服维度评分均降低(P<0.05),且观察组面对维度评分高于对照组、回避和屈服维度评分低于对照组(P<0.05);两组干预前HHI量表各维度得分比较差异无统计学意义(P>0.05),干预后两组HHI量表各维度得分均升高、且观察组HHI量表各维度得分均高于对照组(P<0.05);两组干预前生活质量量表各维度评分比较差异无统计学意义(均P>0.05),干预后两组量表各维度评分均升高、且观察组各维度评分均高于对照组(P<0.05)。结论伴侣参与认知访谈联合希望干预可减轻RSA患者焦虑、抑郁情绪,优化心理应对方式,希望水平和生活质量。

Objective To explore and analyze the influence of partner interview and hope intervention on mental state and coping style of patients with recurrent abortion.Methods A total of 123 patients with recurrent abortion admitted to the Obstetrics and Gynecology Hospital of Fudan University from April 2019 to April 2021 were selected,and were randomly divided into the observation group(62 patients)and the control group(61 patients)by digital table method.The control group was given conventional nursing intervention combined with hope intervention,while the observation group was given partner's participation in cognitive interview combined with hope intervention on the basis of the control group.Two weeks later,Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)were used to evaluate the psychological states of the two groups before and after intervention,the Medical Coping questionnaire(MCMQ)was used to evaluate the psychological coping styles of the two groups,the hope level of the patients was assessed by the Herth Hope Scale(HHI),and the quality of life questionnaire(SF-36)was used to evaluate the quality of life of the two groups.Results There were no significant differences in the HAMA score and HAMD score between the two groups before intervention(P>0.05).After intervention,the scores of the above two scales decreased in both groups,and the scores of both scales in the observation group were lower than those in the control group(P<0.05).Before intervention,there was no significant difference between the two groups in each dimension of coping styles(P>0.05).After intervention,the scores of both groups increased in the face dimension and decreased in the avoidance and submission dimension(P<0.05).In addition,the face dimension score of the observation group was higher than that of the control group,and the avoidance and submission dimension score was lower than that of the control group(P<0.05).There was no significant difference in HHI scores between the two groups before intervention(P>0.05).After intervention,the scores of HHI scale in both groups increased,and the scores of HHI scale in the observation group were higher than those in the control group(P<0.05).There was no significant difference between the two groups in each dimension of quality of life scale before intervention(P>0.05).After intervention,the scores of each dimension of the scale increased in both groups,and the scores of each dimension in the observation group were higher than those in the control group(P<0.05).Conclusions Partner′s participation in cognitive interview combined hope intervention for patients with RSA can relieve the anxiety and depression,optimize psychological coping style,hope level and quality of life.
