血清AQP9、HIF-1α水平与急性缺血性脑卒中出血性转化的相关性分析 被引量:1

摘要 目的探讨血清水通道蛋白9(AQP9)、缺氧诱导因子-1α(HIF-1α)水平与急性缺血性脑卒中出血性转化的相关性。方法本研究选取2019年5月—2022年5月重庆市巴南区人民医院收治的181例急性缺血性脑卒中患者为研究对象。所有患者均进行CT检查,并根据检查结果分为出血性转化组(n=25)及非出血性转化组(n=156)。收集患者一般资料、血清指标、溶栓指标等资料。采用多因素Logistic回归分析影响急性缺血性脑卒中出血性转化的相关因素。结果2组患者性别构成、年龄、身体质量指数(BMI)等一般资料比较,差异均无统计学意义(P>0.05)。出血性转化组患者AQP9、HIF-1α水平高于非出血性转化组,差异均有统计学意义(P<0.05)。2组患者甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)以及高密度脂蛋白胆固醇(HDL-C)水平比较,差异均无统计学意义(P>0.05)。2组患者基线收缩压、基线舒张压比较,差异均无统计学意义(P>0.05)。出血性转化组患者溶栓时间窗长于非出血性转化组,基线美国国立卫生研究院卒中量表(NIHSS)评分高于非出血性转化组,差异均有统计学意义(P<0.05)。多因素Logistic回归分析显示:AQP9、HIF-1α、溶栓时间窗、基线NIHSS评分均为急性缺血性脑卒中患者发生出血性转化的影响因素(P<0.05)。结论影响急性缺血性脑卒中患者发生出血性转化的相关因素包括AQP9、HIF-1α、溶栓时间窗、基线NIHSS评分,临床上应根据以上因素制定相关策略,以降低急性缺血性脑卒中出血性转化的发生率。 Objective This paper aims to investigate the relationship between serum aquaporin-9(AQP9),hypoxia-inducible factor-1α(HIF-1α)levels,and hemorrhagic transformation in acute ischemic stroke.Methods One hundred and eight-one patients with acute ischemic stroke admitted to People's Hospital of Chongqing Banan District from May 2019 to May 2022 were selected as the study participants.All patients underwent CT and were allocated to the hemorrhagic transformation group(n=25)and non-hemorrhagic transformation group(n=156)following the examination results.General data,serum parameters,thrombolytic parameters and other data were collected.Multivariate logistic regression was used to analyze the related influencing factors of hemorrhagic transformation in acute ischemic stroke.Results There was no significant difference in sex composition,age,BMI and other general data between the two groups(P>0.05).Patients in the hemorrhagic transformation group had significantly higher AQP9 and HIF-1αlevels than those in the non-hemorrhagic transformation group(P<0.05).No significant differences were observed in LDL-C,HDL-C,TG,and TC levels between the groups(P>0.05).No significant differences were observed in baseline systolic blood pressure and baseline diastolic blood pressure between the groups(P>0.05).Patients in the hemorrhagic transformation group had a significantly longer thrombolysis time window and a significantly higher baseline NIHSS score than those in the nonhemorrhagic transformation group(P<0.05).Multivariate logistic regression analysis showed that AQP9,HIF-1α,thrombolysis time window,and baseline NIHSS score were all factors for hemorrhagic transformation in patients with acute ischemic stroke(P<0.05).Conclusion The related influencing factors of hemorrhagic transformation in patients with acute ischemic stroke include AQP9,HIF-1α,thrombolysis time window,and baseline NIHSS score.Clinically,relevant strategies should be developed according to the above factors to reduce the incidence of hemorrhagic transformation in acute ischemic stroke.
