临床药师对肺部感染伴肾功能亢进患者的药学服务

Pharmaceutical care by clinical pharmacists for patients with pulmonary infection commorbid with augmented renal clearance

目的介绍临床药师参与1例肺部感染伴肾功能亢进(ARC)患者的抗感染治疗过程,以期为临床治疗提供参考。方法临床药师参与1例肺部感染伴ARC患者抗感染治疗方案的调整,根据药物在患者体内的药代动力学特点,结合国内外相关文献,分析治疗方案并提出建议,提供药学服务。结果临床药师建议被采纳,经用药方案和用药剂量的调整,患者最终好转出院。结论临床药师在临床诊疗过程中,应发挥自身专业优势,协助医师制定个体化给药方案,以更好地促进临床合理用药,保障患者用药的有效性和安全性。

Objective This paper aims to introduce the participation of clinical pharmacists in the anti-infective therapy of a patient with pulmonary infection accompanied by augmented renal clearance(ARC)to provide evidence for clinical treatment.Methods Clinical pharmacists participated in the adjustment of an anti-infective therapy regimen in a patient with pulmonary infection accompanied by ARC.According to the pharmacokinetic changes in this patient,the treatment regimen,combined with relevant domestic and foreign literature,was analyzed and suggestions were put forward to provide pharmaceutical care.Results The clinical pharmacist's suggestion was adopted.After the adjustment of the medication regimen and dosage,the patient was finally improved and discharged.Conclusion In the process of clinical diagnosis and treatment,clinical pharmacists should give full play to their professional advantages and assist physicians in developing individualized dosing regimens to better promote the rational use of drugs in clinical practice and ensure the effectiveness and safety of medication for patients.
