PLGF、sFlt-1、sFlt-1/PLGF比值及TBIL预测子痫前期的临床应用价值

The clinical application value of PLGF,sFlt-1,sFlt-1/PLGF ratio and TBIL in predicting preeclampsia

目的 探究孕妇妊娠早中期血清中胎盘生长因子(PLGF)、可溶性血管内皮生长因子受体-1(s Flt-1)、s Flt-1/PLGF比值及总胆红素(TBIL)水平对子痫前期疾病发生的预测价值,为子痫前期的临床防治提供切实有效的实验室依据。方法 选取福建省立金山医院2019年11月至2022年8月收治的173例子痫前期的孕妇作为研究组,另外选取同时期进行常规孕检的健康孕妇175名作为对照组,开展回顾性研究。比较两组血清中PLGF、s Flt-1、s Flt-1/PLGF比值及TBIL水平的高低,分析血清中PLGF、s Flt-1、s Flt-1/PLGF比值及TBIL单项指标检测对预测子痫前期的临床应用价值。结果 研究组中血清PLGF和TBIL的水平低于对照组,研究组中的s Flt-1水平和s Flt-1/PLGF比值高于对照组(P <0.05)。接受者操作特征(ROC)曲线分析:PLGF、s Flt-1、s Flt-1/PLGF比值及TBIL对子痫前提的诊断曲线下面积(AUC)结果分别为0.923、0.927、0.942、0.890,四项指标均对子痫前期具有预测意义(P <0.05)。结论 妊娠早中期检测血清PLGF、s Flt-1、s Flt-1/PLGF比值及TBIL对子痫前期的诊断均具有良好的预测性。

Objective To investigate the predictive value of placental growth factor(PLGF),soluble fms-like tyrosine kinase receptor-1(sFlt-1),sFlt-1/PLGF ratio,and total bilirubin(TBIL)levels in serum of pregnant women in the early and middle stages of pregnancy for the occurrence of preeclampsia,and to provide practical and effective laboratory basis for the clinical prevention and treatment of preeclampsia.Methods A total of 173 pregnant women with preeclampsia admitted to Fujian Provincial Jinshan Hospital from November 2019 to August 2022 were selected as the study group.In addition,A total of 175 healthy pregnant women who underwent routine pregnancy examinations during the same period were selected as the control group for a retrospective study.The levels of PLGF,sFlt-1,sFlt-1/PLGF ratio,and TBIL in the serum of two groups were compared,and the clinical application value of PLGF,sFlt-1,sFlt-1/PLGF ratio,and TBIL single detection in predicting preeclampsia were analyzed.Results The levels of PLGF and TBIL in serum in the study group were lower than those in the control group,while the levels of sFlt-1 and sFlt-1/PLGF ratio in the study group were higher than those in the control group(P<0.05).Receiver operating characteristic curve(ROC)analysis:The results of PLGF,sFlt-1,sFlt-1/PLGF ratio,and TBIL for the area under the curve(AUC)of diagnosis of preeclampsia were 0.923,0.927,0.942,and 0.890,respectively.All four indexes had predictive significance for preeclampsia(P<0.05).Conclusion The detection of PLGF,sFlt-1,sFlt-1/PLGF ratio,and TBIL in serum in the early and middle stages of pregnancy has good predictive value for the diagnosis of preeclampsia.
