中医传统运动对脑卒中患者认知功能影响的meta分析

Meta analysis of the impact of traditional Chinese medicine exercise on cognitive function in stroke patients

目的系统评价中医传统运动对脑卒中患者认知功能的干预效果。方法检索PubMed、Cochrane Library、Embase、Web of Science、中国知网(CNKI)、VIP、中国生物医学文献数据库(CBM)、万方数据库(WANFANG)中有关中医传统运动治疗卒中后认知障碍的随机对照研究,时间为建库至2023年2月1日。采用Cochrane风险偏倚评估工具评价文献质量,使用RevMan 5.3软件进行meta分析。结果共纳入9篇文献。meta分析结果提示:中医传统运动能够改善脑卒中患者的整体认知功能MoCA评分[MD=2.36,95%CI(2.09,2.63),P<0.00001]、执行功能TMT-A评分[MD=-39.06,95%CI(-46.53,-31.60),P<0.00001]、即刻记忆[MD=1.28,95%CI(0.98,1.58),P<0.00001]、短时延迟记忆[MD=1.23,95%CI(0.90,1.55),P<0.00001]和长时延迟记忆[MD=1.15,95%CI(0.87,1.43),P<0.00001]。纳入文献均未报告发生不良事件。结论中医传统运动可有效改善脑卒中患者的整体认知功能、执行功能和记忆功能,但仍需高质量的研究进一步验证。

Objective To systematically evaluate the intervention effect of traditional Chinese medicine exercise on cognitive function in stroke patients.Methods The randomized controlled trials of traditional Chinese medicine exercise for cognitive impairment after therapy for stroke was retrieved from PubMed,Cochrane Library,Embase,Web of Science,China national knowledge infrastructure(CNKI),VIP,Chinese biomedical literature database(CBM),and wanfang database(WANFANG)from the establishment of the database to February 1,2023.The Cochrane risk bias tool was used to evaluate the quality of the literature,and the RevMan 5.3 software was used for meta analysis.Results A total of 9 articles were included.The meta-analysis results suggested that traditional Chinese medicine exercise could improve the overall cognitive function MoCA score of stroke patients(MD=2.36,95%CI[2.09,2.63],P<0.00001),executive function TMT-A score(MD=-39.06,95%CI[-46.53,-31.60],P<0.00001),immediate memory(MD=1.28,95%CI[0.98,1.58],P<0.00001),short-term delayed memory(MD=1.23,95%CI[0.90,1.55],P<0.00001)and long-term delayed memory(MD=1.15,95%CI[0.87,1.43],P<0.00001).No adverse events were reported in the included literature.Conclusion Traditional Chinese medicine exercises could effectively improve the overall cognitive function,executive function,and memory function of stroke patients,but high-quality research is still needed for further verification.
