湿润烧伤膏联合复方黄柏液在合并糖尿病的肛周脓肿患者术后创面中的应用效果分析

Application Effect of MEBO Combined with Fufang Huangbai Ye in the Postoperative Wound of Perianal Abscess in Patients with Diabetes

目的探讨湿润烧伤膏联合复方黄柏液在合并糖尿病的肛周脓肿患者肛周脓肿切开引流术后创面中的应用效果。方法选取2020年6月至2022年6月湖北省中医院收治的拟行肛周脓肿切开引流术治疗的80例合并糖尿病的肛周脓肿患者作为研究对象,按照随机数表法将其随机分为观察组(40例)与对照组(40例),观察组患者肛周脓肿切开引流术后创面采用湿润烧伤膏联合复方黄柏液治疗,对照组患者肛周脓肿切开引流术后创面采用复方黄柏液治疗,对比观察两组患者血清炎症因子水平及创面疼痛、渗液、肉芽组织生长与水肿情况。结果术后第7天,观察组患者血清白细胞介素(IL)-6、肿瘤坏死因子-α(TNF-α)水平均明显低于对照组(t=6.764、2.656,P<0.001、P=0.010),IL-10水平明显高于对照组(t=4.290,P<0.001);术后第3、7天,观察组患者视觉模拟评分法(VAS)评分均明显低于对照组(t=2.517、7.365,P=0.014、P<0.001);术后第7天,观察组患者创面渗液、肉芽组织生长和水肿评分均明显低于对照组(t=4.002、3.910、4.246,P均<0.001)。结论湿润烧伤膏联合复方黄柏液治疗合并糖尿病的肛周脓肿患者术后创面,能够有效降低机体炎症反应程度,减轻术后创面疼痛及水肿,加快肉芽组织生长。

Objective To study the effect of applying MEBO and Fufang Huangbai Ye in the wound of incision and drainage for perianal abscess in patients with diabetes.Methods 80 patients with diabetes admitted into Hubei Provincial Hospital of TCM for incision and drainage of perianal abscess between June 2020 and June 2022 were enrolled as research subjects,and divided into study group(n=40)and control group(n=40)using the random number table.After the incision and drainage for perianal abscess,the postoperative wounds of patients in the study group were treated with MEBO plus Fufang Huangbai Ye,while the wounds of patients were treated with Fufang Huangbai Ye alone in the control group.The levels of serum inflammatory factors,wound pain,exudates,growth of granulation tissues and edema of patients were compared between the two groups.Results On day 7 after surgery,the levels of serum interleukin(IL)-6,tumor necrosis factor-α(TNF-α)of patients were obviously lower(t=6.764 and 2.656,P<0.001,P=0.010),and the levels of IL-10 was significantly higher(t=4.290,P<0.001)in the study group compared with the control group.Respectivelyon day 3 and 7 after surgery,the visual analogue scale(VAS)scores of patients in the study group were both lower than that in the control group(t=2.517 and 7.365,P=0.014,P<0.001).On day 7 after surgery,the scores of wound exudates,growth of granulation tissues and edema were all markedly lower in the study group compared with the control group(t=4.002,3.910 and 4.246,all P<0.001).Conclusion The combined application of MEBO and Fufang Huangbai Ye in the postoperative wound of perianal abscess in patients with diabetes can significantly decrease the inflammatory responses of patients,alleviate postoperative wound pain and edema,and accelerate the growth of granulation tissues.
