临床护理人员对围术期病人疼痛管理知信行问卷的编制与信效度检验

Development of clinical nurses' knowledge ⁃ attitude ⁃ practice questionnaire on pain managementof perioperative patients and its reliability and validity test

目的:编制临床护理人员对围术期病人疼痛管理知信行问卷,并检验问卷信效度。方法:检索围术期疼痛管理相关证据,以知信行理论框架构建问卷条目池,通过小组讨论、专家函询、预调查修订问卷条目。选取某三级甲等医院手术科室957名护理人员进行问卷调查,以检验问卷信效度。结果:共进行了2轮专家函询,专家积极系数为90%、100%,权威系数为0.895,Kendall和谐系数分别为0.192,0.250(均P<0.01)。临床护理人员对围术期病人疼痛管理知信行问卷包括知识、态度和行为3个维度,共41个条目。问卷Cronbach'sα系数为0.984,3个维度Cronbach'sα系数为0.958,0.983,0.983;折半系数为0.888,3个维度折半系数分别为0.883,0.956,0.954。条目水平的内容效度指数(I-CVI)为0.800~1.000,平均量表水平的内容效度指数(S-CVI/Ave)为0.985;探索性因子分析提取3个公因子,累计方差贡献率为77.60%。结论:编制的临床护理人员对围术期病人疼痛管理知信行问卷信效度良好,可用于调查护理人员对围术期病人疼痛管理的知识、态度和行为,为临床管理者制定和完善护理人员疼痛相关培训策略提供理论指导。

Objective:To develop clinical nurses'knowledge⁃attitude⁃practice questionnaire on pain management of perioperative patients,and to test its reliability and validity.Methods:The relevant evidence of perioperative pain management was retrieved.The questionnaire item pool was constructed based on the theoretical framework of knowledge,attitude and practice(KAP),and the questionnaire items were revised through group discussion,expert correspondence and pre⁃investigation.A total of 957 clinical nurses from a tertiary grade A hospital were selected for questionnaire survey by convenience sampling method,in order to test the reliability and validity of the questionnaire.Results:Two rounds of expert consultations were conducted.The positive coefficients of experts in the two rounds of consultations were 90%and 100.00%.The authority coefficients of experts was 0.895.The values of Kendall's W were 0.192 and 0.250,respectively(all P<0.01).The questionnaire included following 3 dimensions:knowledge,attitude and practice,involving 41 items.The Cronbach'sαcoefficient was 0.984.And the Cronbach'sαcoefficient of each dimension were 0.958,0.983 and 0.983.The Split⁃Half coefficient of the total scale was 0.888.The Split⁃Half coefficient of each dimension were 0.883,0.956 and 0.954,respectively.The item⁃content validity index(I⁃CVI)ranged from 0.800 to 1.000,and the average scale⁃content validity index(S⁃CVI/Ave)was 0.985.Exploratory factor analysis extracted 3 common factors,and the cumulative contribution rate of variance was 77.60%.Conclusions:The clinical nurses'knowledge⁃attitude⁃practice questionnaire on pain management of perioperative patients had acceptable reliability and validity,which could be used to investigate nurses'knowledge,attitude and practice on pain management of perioperative patients,and to provide theoretical guidance for clinical managers to formulate and improve pain⁃related training strategies for nursing staff.
