IVIM-DWI 在腰椎间盘突出症合并神经根病疼痛评估中的应用

摘要 目的 探讨体素内不相干运动扩散加权成像(IVIM-DWI)在腰椎盘突出症合并神经根病(LDHR)疼痛评估中的作用。方法 选取92例LDHR患者分为L5观察组、S1观察组以及30例健康志愿者作为对照组,三组人员进行3DFIESTA序列成像、IVIM-DWI及同层面常规T_2序列成像,测量脊后根神经节获得定量参数ADC、D、D^(*)、F的数值,通过ODI量表获得观察组疼痛评分,分析定量参数和ODI评分之间的关系。结果 1)观察组与对照组脊背根神经节的ADC、D、F组间差异有统计学意义(P<0.05),D^(*)组间差异无统计学意义(P>0.05);2)观察组ADC、D与ODI评分均呈中度相关。结论 IVIM-DWI参数ADC、D可用于LDHR患者的疼痛评估。 Objective To explore the role of intravoxel incoherent motion-diffusion-weighted imaging(IVIM-DWI)in pain assessment of lumbar disc herniation with radiculopathy(LDHR).Methods 92 patients with LDHR were divided into L5 observation group,S1 observation group and 30 healthy volunteers were selected as control group.3D-FIESTA sequence imaging,IVIM-DWI and conventional T 2 sequence imaging were performed in the three groups.The quantitative parameters including ADC value,D value,D*value and F value were obtained by measuring the posterior root ganglion on MRI.The pain score of the observation group was obtained by ODI scale,and the relationship between quantitative parameters and ODI score was studied.Results 1)The ADC,D,F values of dorsal root ganglion in the observation group were significantly different from those in the control group,but the D*values had no significant difference;2)The value of ADC and D in the observation group were all moderately correlated with ODI score.Conclusion IVIM-DWI parameters including ADC value and D value can be used to evaluate pain in patients with LDHR.
