Anti-cancer drugs and cardiovascular disease in the elderly

心血管疾病和恶性肿瘤是威胁人类健康的两类主要疾病,心血管疾病合并恶性肿瘤的临床情境在老年患者中愈发常见。随着肿瘤治疗技术的飞速进步,肿瘤患者预后明显改善的同时,很多肿瘤患者发生心血管事件甚至死亡,而肿瘤治疗相关的心血管毒性被认为是一项很重要的影响因素。如何在抗肿瘤治疗获益的基础上减少或避免相关的心血管毒性,是所有肿瘤科医生和心血管病医生共同面临的挑战。文章就抗肿瘤药物的心血管毒性、抗肿瘤药物心血管毒性监测与防治以及抗肿瘤药物与心血管药物的相互作用进行总结,并指出抗肿瘤药物心血管毒性的防治重点在于风险评估、提前预警、优化治疗方案并及早识别不良反应。

Cardiovascular diseases and cancers are two main types of diseases that threaten human health.The clinical scenario of cardiovascular diseases combined with malignant tumors is becoming increasingly common in elderly patients.With the progress of cancer therapeutics,the prognosis of patients with cancer has been remarkably improved.Meanwhile,many patients with cancer have experienced adverse cardiac events or eren death and cancer treatment related cardiovascular toxicity is considered a critical risk factor.Alleviating or aroiding cardiovascular toxicity on the basis of benefits from cancer treatment is a common challenge for both oncologists and cardiologists.This review summarizes the cancer treatment related cardiovascular toxicity,prevention,treatment and monitoring of the toxicity and the interaction between anti-cancer drugs and cardiovascular drugs.It is suggested that the prevention and treatment of cancer related cardiovascular toxicity should focus on risk evaluation,precaution prior to treatment,optimization of treatment protocols,and early recognition of the cardiovascular toxicity.
