^(18)F-FDG PET/CT Combined with Enhanced CT in Diagnosis of Pulmonary Mixed Ground-Glass Nodules

摘要 目的探讨^(18)F-FDG PET/CT联合增强CT对肺部混合磨玻璃结节(mGGN)的诊断价值。资料与方法回顾性分析2019年1月—2021年12月于南通大学第四附属医院进行PET/CT和增强CT检查的57例单发mGGN,根据病理亚型及临床意义不同将mGGN分为A组(炎性结节组,9例)、B组(腺体前驱病变-微浸润腺癌组,21例)及C组(浸润性腺癌组,27例),分析PET/CT的最大标准化摄取值(SUVmax)、增强CT强化净增值(ΔCT)和肿瘤微血管CT成像征对mGGN的诊断效能。结果A组与B组比较:SUVmax联合ΔCT及肿瘤微血管CT成像征预测mGGN的效能优于单独PET/CT参数及增强CT参数,差异有统计学意义(P=0.003、0.026)。B组与C组比较:SUVmax联合ΔCT及肿瘤微血管CT成像征预测mGGN的效能优于PET/CT参数,但差异无统计学意义(P>0.05);SUVmax联合ΔCT及肿瘤微血管CT成像征预测mGGN的效能优于增强CT参数,差异有统计学意义(P=0.048)。PET/CT参数、增强CT参数、SUVmax联合ΔCT及肿瘤微血管CT成像征对A组和B组的诊断准确度分别为40.00%、76.67%、93.33%;对B组和C组的诊断准确度分别为77.08%、45.83%、79.17%。结论PET/CT联合增强CT对mGGN的诊断效能优于两者单一诊断,且有助于提高mGGN的诊断准确度。 Purpose To investigate the diagnostic value of^(18)F-FDG PET/CT combined with enhanced CT in mixed ground-glass opacity nodule(mGGN).Materials and Methods Fifty-seven patients with single mGGN who underwent PET/CT and enhanced CT examination in the Fourth Affiliated Hospital of Nantong University from January 2019 to December 2021 were retrospectively analyzed,according to different pathological subtypes and clinical significance,mGGN was divided into group A(inflammatory nodule,9 cases),group B(precursive glandular disease to minimally invasive adenocarcinoma,21 cases)and group C(invasive adenocarcinoma,27 cases),and the diagnostic value of SUVmax,enhanced net value added(ΔCT)and tumor microvascular CT imaging sign for mGGN was analyzed.Results Compared between group A and group B,the diagnostic efficacy of SUVmax combined withΔCT and tumor microvascular CT imaging sign in predicting mGGN was statistically better than that of PET/CT parameter or enhanced CT parameter(P=0.003,0.026).Compared between group B and group C,the diagnostic efficacy of SUVmax combined withΔCT and tumor microvascular CT imaging sign in predicting mGGN was better than that of PET/CT parameter,but showed no statistical difference(P>0.05);the diagnostic efficacy of SUVmax combined withΔCT and tumor microvascular CT imaging sign in predicting mGGN was statistically better than that of enhanced CT parameter(P=0.048).The diagnostic accuracy of PET/CT parameter,enhanced CT parameter and combined parameter in group A and B were 40.00%,76.67%and 93.33%,respectively;the diagnostic accuracy of the above parameters in group B and group C were 77.08%,45.83%and 79.17%,respectively.Conclusion The diagnostic efficiency of PET/CT combined with enhanced CT for mGGN is better than that of PET/CT or enhanced CT alone,and it is beneficial to improve the diagnostic accuracy of mGGN.
