养阴清肺膏治疗肺肾阴虚型稳定期慢性阻塞性肺病的临床疗效

Clinical study on Yangyin Qingfei Gao in treating stable chronic obstructive pulmonary disease of lung and kidney Yin deficiency type

目的探讨养阴清肺膏治疗肺肾阴虚型稳定期慢性阻塞性肺病(COPD)的临床疗效及对患者肺及免疫功能的影响。方法将我院2018年1月到2021年12月收治的120例肺肾阴虚型稳定期COPD患者纳入此次研究,行数字双盲法分为对照组与研究组,每组患者各60例,对照组给予常规治疗,研究组在对照组治疗基础上添加养阴清肺膏治疗,连续治疗3个月,比较两组患者临床疗效、治疗前后肺功能[肺活量(FVC)、第1秒用力呼气容积(FEV 1)、FEV 1/FVC、总肺气肿容积(TEV)、总肺容积(TLV)和肺气肿指数(EI)]、COPD评估测试(CAT)评分、中医证候评分和免疫功能水平[免疫球蛋白IgG、IgA、IgM和辅助性T细胞1(Th1)/Th2比值]。结果治疗后研究组总有效率显著高于对照组;治疗前两组患者肺功能、CAT评分、中医证候评分和和免疫功能水平没有显著性差异,治疗后研究组FVC、FEV 1、FEV 1/FVC、IgA、IgM、IgG和Th1/Th2水平均显著高于对照组,而CAT评分、中医证候评分、TEV、TLV和EI水平均显著低于对照组。结论养阴清肺膏治疗肺肾阴虚型稳定期COPD疗效确切,可有效增强患者机体免疫力,调节免疫平衡,显著改善患者的临床症状和生活质量,具有较高的临床应用价值。

Objective To investigate the clinical efficacy of Yangyin Qingfei Gao in the treatment of stable chronic obstructive pulmonary disease(COPD)of lung and kidney Yin deficiency type and its influence on the lung and immune function of patients.Methods From January 2018 to December 2021 were treated 120 cases of patients with stable COPD of lung and kidney Yin deficiency type in this study,digital double blind method were divided into control group and study group,each group of patients with 60 cases,give control group routine treatment,the study team added YangYin QingFei Gao therapy from the other side,compared to patients with clinical efficacy,lung function[forced vital capacity(FVC),forced expiratory volume in one second(FEV 1),FEV 1/FVC,total emphysema volume(TEV),total lung volume(TLV)and emphysema index(EI)],COPD Assessment Test(CAT),TCM syndrome score and immunity levels[immunoglobulin IgG,IgA,IgM and T helper 1(Th1)/Th2]before and after treatment.Results After treatment,the total effective rate of the study group was significantly higher than that of the control group before treatment,there was no significant difference in lung function index and immune function index(P>0.05).There was no significant difference in pulmonary function,CAT score,TCM syndrome score and immune function between the two groups before treatment.After treatment,the levels of FVC,FEV 1,FEV 1/FVC,IgA,IgM,IgG and Th1/Th2 in the study group were significantly higher than those in the control group,while CAT score,TCM syndrome score,TEV,TLV and EI levels were significantly lower than those in the control group.Conclusion Yangyin Qingfei Gao has a definite effect on stable COPD of lung and kidney Yin deficiency type,which can effectively enhance the patient′s immune system,regulate immune balance,significantly improve the patient′s clinical symptoms and quality of life,and has high clinical application value.
