摘要
目的通过建立前置审核预警机制,促进医院高浓度电解质的合理使用,降低潜在医疗风险。方法在处方前置审核系统规则库中创建高浓度电解质预警规则,通过实时调取住院患者电解质检查结果,对临床高浓度电解质的使用进行实时预警。结果建立预警机制后,以氯化钾注射液为例,共预警7277次,占氯化钾注射液医嘱总数的23.84%,其中适应证预警2961次,占总预警数的40.69%,溶媒配伍预警518次,占总预警数的7.12%,给药途径预警共3503条,占总预警数的48.14%,给药剂量预警共295条,占总预警数的7.34%。结论通过建立住院患者高浓度电解质使用预警机制,可提示降低高浓度电解质用药差错、消除潜在用药风险,保障患者用药安全。
Objective To promote the rational use of high-concentration electrolytes and reduce potential risks of malpracticing in hospital by development of an early alert module in the pre-prescription audit system.Methods The rules for early alert against use of high-concentration electrolytes were built up in a rule dataset of the pre-prescription audit system.Alerts for high-concentration electrolytes in clinical use were prompted by the system via real-time retrieval of laboratory data for electrolyte levels in the inpatients.Results After establish-ment of the early alert module,in the exemplary case of potassium chloride(KCl)injections,there were 7277 pop-ups of early alert,accounting for 23.84%of all KCL prescriptions.Of these alerts,there were 2961 prompted for the medical indications(40.69%),518 for compatibility of the solvents(7.12%),3503 for the administration routes(48.14%),and 295 for the doses(7.34%).Conclusion Establishment of an early alert module against the use of high-concentration electrolytes in inpatients can prompt for reducing malpractice in using high-concentra-tion electrolytes,avert potential risks and ensure patient safety with the medications.
