《古今名医临证金鉴·五官科卷》治疗复发性口腔溃疡用药规律研究

Study on the Medication Rules in the Treatment of Recurrent Oral Ulcer in The ENT Section of The Golden Testimony of Clinical Evidence of Ancient and Modern Famous Doctors

目的分析古今医家治疗复发性口腔溃疡的组方用药规律。方法收录《古今名医临证金鉴·五官科卷》中治疗复发性口腔溃疡(ROU)的内服处方,借助中医传承辅助平台(V3.0)、IBM SPSS Modeler18.1和IBM SPSS Statistics 25.0软件对纳入中药分别进行用药频次、性味归经、熵聚类、关联规则以及因子分析。结果最终收录有效处方102首,涉及中药176味,其中使用频数较高的中药是甘草、白术、地黄、茯苓等,归经以脾、肺、心、胃经为先,药性偏寒、温、平,药味分布以甘、苦、辛居多。中药使用频数最高的是补虚药,其次为清热药、利水渗湿药等,常用药对有“白术-甘草”、“地黄-甘草”、“茯苓-甘草”等,聚类分析得到三组核心药物组合,中药因子分析获得11个公因子。结论古今医家治疗ROU以补虚为主,重视顾护脾胃,辅以清热、利水渗湿、理气等,攻补兼施。

Objective:To analyze the rules of prescription and medication in the treatment of recurrent oral ulcer.Methods:The internal prescription for the treatment of recurrent oral ulcer(ROU)in the The ENT Section of The Golden Testimony of Clinical Evidence of Ancient and Modern Famous Doctors were included and the medication frequency,properties and taste channel tropism,entropy clustering,association rules and factors were analyzed respectively by using TCM inheritance assistance platform(V3.0),IBM SPSS Modeler18.1 and IBM SPSS Statistics 25.0 software.Results:102 effective prescriptions were included,involving 176 TCM flavor herbs,and among them,the most frequently used herbs were licorice,atractylodis,rehmannia and poria.The channel tropism took spleen,lung,heart and stomach for the first,the properties tended toward cold,warm and moderate,and the taste distribution was mostly sweet,bitter and pungent.The most frequently used herbs were defiency-tonifying,followed by heat-clearing and damp-clearing.The commonly used pairs were atractylodis-licorice,rehmannia-licorice,poria-licorice,etc.Three groups of core herb combinations were obtained by cluster analysis,and 11 common factors were obtained by Chinese medicine factor analysis.Conclusion:The ancient and modern doctors mainly treat ROU by taking deficiency-tonifying as the principal thing and stressing on the protection of spleen and stomach,supplemented by clearing heat,expelling damp to promote diuresis and regulating qi,with tonification and purgation in combination.
