当归芎艾汤联合低剂量缩宫素促宫颈成熟临床疗效观察

Clinical Observation of Danggui Xiongai Decoction Combined with Low Dose Oxytocin to Promote Cervical Ripening

目的探讨当归芎艾汤联合低剂量缩宫素在加速宫颈成熟中的应用价值。方法收集2022年1月至2022年7月于都县人民医院住院的60例产妇患者作为研究对象,随机数字表法分为对照组与试验组各30例。对照组采用每日静滴2.5 IU缩宫素注射液促进宫颈成熟,试验组先让患者口服医院熬制的当归芎艾汤剂治疗3 d,再每日静滴2.5 IU缩宫素注射液促进宫颈成熟。统计分析两组患者用药前及用药后宫颈成熟度(Bishop)评分的变化,两组患者之间的差异;监测记录两组产妇不同分娩方式例数,两组产妇平均临产时间、破膜时间及总产程时长;不良反应发生情况。结果试验组促宫颈成熟有效率为93.33%,对照组为80.00%,差异有统计学意义(P<0.05);两组产妇用药后宫颈Bishop评分均有所提高,但是试验组评分提高较为明显,差异有统计学意义(P<0.05);试验组中顺产例数较对照组多,且临产时间、破膜时间均短于对照组,提示试验组数据总体优于对照组,差异有统计学意义(P<0.05)。两组产妇总产程差异无统计学意义(P>0.05),且使用过程中两组产妇均未发生子宫过度刺激、子宫强直性收缩、药物过敏、胎儿异常等不良事件。结论当归芎艾汤联合低剂量缩宫素促进产妇宫颈成熟安全、有效,能明显提升阴道分娩率。

Objective To analyze the application value of danggui xiongai decoction combined with low dose oxytocin in accelerating cervical ripening.Methods 60 cases of parturients hospitalized in Yudu County People's Hospital from January 2022 to July 2022 were collected as research objects and randomly divided into control group and experimental group with 30 cases each.The control group was treated with 2.5 IU oxytocin injection on daily intravenous drip to promote cervical maturation,while the experimental group was treated with danggui xiongai decoction prepared by the hospital for 3 days,and then 2.5 IU oxytocin injection on daily intravenous drip to promote cervical maturation.The Bishop scores of the two groups of patients before and after medication were recorded;monitor and record the number of cases of different delivery methods in two groups of parturients;compare the average delivery time,rupture time,and total duration of labor between the two groups of postpartum women,the occurrence of adverse reactions.Results The effective rate of promoting cervical maturation in the experimental group was 93.33%,while in the control group it was 80.00%,with a statistically significant difference(P<0.05);both groups of postpartum women showed an improvement in the Bishop score of the cervix after medication,but the experimental group showed a significant improvement in the score,with a statistically significant difference(P<0.05);the number of cases of spontaneous labor in the experimental group was higher than that in the control group,and the delivery time and rupture time were shorter than those in the control group.The overall data of the experimental group was better than that of the control group,with a statistically significant difference(P<0.05).There was no significant difference in the total labor process between the two groups(P>0.05),and there were no adverse events such as uterine hyperstimulation,uterine rigidity contraction,drug allergy,fetal abnormality,etc.Conclusion Danggui xiongai decoction combined with low dose oxytocin is safe and effective in promoting cervical ripening,can significantly increase the vaginal delivery rate.
