老年慢性病患者居家用药现状调查研究及对策

Investigation and Countermeasures on the Status Quo of Home Medication for Elderly Patients with Chronic Diseases

目的调查研究老年慢性病患者居家用药现状,探讨有效对策以提高老年慢性病患者居家用药的安全性和有效性。方法以社区为单位使用定性和定量方法,自制问卷,对老年慢性病患者居家用药的现状进行调查研究。本次调查共发放问卷300份,涵盖老年慢性病患者疾病共存、服药种类、用药依从性、药物不良反应、药物监测等方面,以了解老年慢性病患者居家用药的安全性和有效性。结果调查问卷有效回收288份,有效回收率为96%(288/300);统计有效回收问卷显示,调查对象受教育程度低,多病共存,用药种类繁多,依从性不佳比例高。此外,调查还发现,有43.75%的患者未定期检测血药浓度等。结论老年慢性病患者居家用药存在着一系列问题,如依从性不佳、药物监测不足等。有效对策建议包括优化用药处方、加强用药宣教、建立老年慢性病患者健康档案并定期随访、保障家庭支持系统、互联网医院等措施,以减少药物不良反应/事件的发生,保证老年慢性病患者居家用药的安全。

Objective This paper mainly investigates the current situation of home medication for elderly patients with chronic diseases,and explores effective countermeasures to improve the safety and effectiveness of home medication for elderly patients with chronic diseases.Methods This study uses qualitative and quantitative methods in the community as a unit,self-made questionnaire to investigate and study the current situation of home medication for elderly patients with chronic diseases.The questionnaire survey covered the coexistence of diseases,types of medication,medication compliance,adverse drug reactions,and drug monitoring in elderly patients with chronic diseases,in order to understand the safety and effectiveness of home medication for elderly patients with chronic diseases.Results 288 were effectively recovered,and the effective recovery rate was 96%(288/300);the data of the effectively recovered questionnaires were sorted out,counted and analyzed,and the survey results showed that the survey respondents had low education and many Co-existing diseases,a wide variety of medications,the proportion of non-compliance was high,the survey results also found that 43.75%of elderly patients with chronic diseases did not regularly detect blood drug concentrations.Conclusion There are a series of problems in home medication for elderly patients with chronic diseases,such as poor compliance and insufficient drug monitoring.Some effective countermeasures and suggestions are put forward,including optimizing drug prescriptions,strengthening drug education,establishing health records for elderly patients with chronic diseases and regular follow-up,ensuring family support systems,Internet hospitals and other measures to improve the rationality of drug use for elderly patients with chronic diseases.Reduce the occurrence of adverse drug reactions/events,and ensure the safety of home medication for elderly patients with chronic diseases.
