疏肝益气解郁方治疗癌症相关性抑郁70例疗效观察

Therapeutic effects of Shugan Yiqi Jieyu decoction on 70 cases of cancer related depression

目的:观察对比自拟方疏肝益气解郁方与临床常用抗抑郁药盐酸帕罗西汀片治疗癌症相关性抑郁的临床疗效。方法:选取2021年10月―2022年4月在秦皇岛市中医医院肿瘤科门诊及病房治疗的70例癌症相关性抑郁患者,采用随机数字表法将其分为治疗组和对照组,每组35例,并确认两组的基本资料具有可比性。治疗方面,对照组在肿瘤内科常规治疗(手术、放化疗、免疫治疗、靶向治疗、对症治疗等)基础上予盐酸帕罗西汀片口服;治疗组则在肿瘤内科常规治疗基础上予自拟中药方疏肝益气解郁方口服进行治疗,每7日为一个疗程,两组均治疗6个疗程,并分别于治疗前后对抑郁自评量表(Self-rating Depression Scale,SDS)评分和汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)评分,以及治疗后的有效率进行比较。结果:两组治疗后SDS评分和HAMD评分较治疗前均有明显改善,且治疗组不劣于对照组,差异有统计学意义(P<0.05);治疗组总有效率为86%(30/35),高于对照组的77%(27/35),但差异无统计学意义(P>0.05)。结论:疏肝益气解郁方可有效改善癌症相关性抑郁患者的抑郁状态,其效果不劣于临床常用药盐酸帕罗西汀片且未见明显毒副反应,疗效显著,值得用作临床参考。

Objective:To observe and compare the clinical efficacy of self-designed Shugan Yiqi Jieyu decoction(疏肝益气解郁方)and commonly used clinical antidepressant paroxetine hydrochloride tablets on cancer related depression(CRD).Methods:A total of 70 patients treated in the Outpatient Department and Ward of the Department of Oncology in Qinhuangdao Hospital of Traditional Chinese Medicine from October 2021 to April 2022 were selected and divided into the reatment group and the control group with random number table method,with 35 cases in each group.The basic data in the two groups were confirmed to be comparable.In terms of treatment,the control group was given paroxetine hydrochloride tablets orally on the basis of conventional medical treatment in oncology(surgery,chemoradiotherapy,immunotherapy,targeted therapy,symptomatic therapy,etc.).The treatment group was given oral administration of self-designed Shugan Yiqi Jieyu decoction on the basis of conventional medical treatment in oncology.Each 7 days was a course of treatment,and both groups were treated for 6 courses of treatment.The efficacy of self-rating Depression scale(SDS scale)and Hamilton Depression Scale(HAMD scale)before and after treatment,as well as the effective rate after treatment were compared.Results:The SDS and HAHD scores in the two groups were significantly ameliorated compared with those before treatment,and the treatment group was not inferior to the control group,and the difference was statistically significant(P<0.05),the total effective rate in the treatment group was 86%(30/35),which was higher than 77%(27/35)in the control group,,but the difference was not statistically significant(P>0.05).Conclusion:Shugan Yiqi Jieyu decoction can effectively ameliorate the depressive state of patients with cancer related depression,and its effect is not inferior to paroxetine hydrochloride tablets,which are commonly used in clinical use,and no obvious toxic and side effects are observed.The efficacy is significant,and it is worthy of clinical reference.
