CGF+同种异体骨颗粒混合充填骨腔联合覆盖生物膜在牙源性颌骨囊肿骨增量术中的应用

摘要 目的:探究浓缩生长因子(concentrated growth factor,CGF)+同种异体骨颗粒混合充填骨腔联合覆盖生物膜在牙源性颌骨囊肿骨增量术中的应用效果。方法:选取2021年7月—2022年12月上饶市人民医院口腔科收治的60例牙源性颌骨囊肿需术中进行骨增量的患者,按照随机数字表法分为观察组(n=30)和对照组(n=30)。观察组采用CGF+同种异体骨颗粒混合充填骨腔后覆盖生物膜,对照组采用CGF+同种异体骨颗粒混合充填骨腔。比较两组临床疗效、术后感染情况、丧失功能牙术后恢复情况及温哥华瘢痕量表(Vancouver scar scale,VSS)。结果:观察组治疗总有效率(90.00%)高于对照组(66.67%)(P<0.05)。术后3个月,观察组轻度感染1例,重度感染3例;对照组轻度感染3例,重度感染10例,两组感染情况比较,差异有统计学意义(P<0.05)。观察组患牙恢复情况优于对照组(P<0.05)。术后1、2、4周,观察组VSS评分均低于对照组(P<0.05)。结论:CGF+同种异体骨颗粒混合充填骨腔联合覆盖生物膜可提高牙源性颌骨囊肿骨增量术治疗的效果,促进患牙恢复,降低术后感染发生率,抑制术后瘢痕产生。 Objective:To investigate the application effect of mixed filling of bone cavity with concentrated growth factor(CGF)and allogeneic bone particles combined with biofilm covering in bone augmentation for odontogenic cysts.Method:A total of 60 patients with odontogenic cysts who were admitted to the Stomatology Department of Shangrao People's Hospital from July 2021 to December 2022,and needed bone augmentation during operation were selected and divided into observation group(n=30)and control group(n=30)according to random number table method.The observation group was treated with biofilm covering after mixed filling of bone cavity with CGF and allogeneic bone particles,while the control group was treated with mixed filling of bone cavity with CGF and allogeneic bone particles.Clinical efficacy,postoperative infection,postoperative recovery of teeth with loss of function,and Vancouver scar scale(VSS)scores were compared between the two groups.Result:The total treatment effective rate in the observation group(90.00%)was higher than that in the control group(66.67%)(P<0.05).Three months after operation,there was 1 case of mild infection and 3 cases of severe infection in the observation group,3 cases of mild infection and 10 cases of severe infection in the control group,the difference in infection between the two groups was statistically significant(P<0.05).The recovery of affected teeth in the observation group was better than that in the control group(P<0.05).The VSS scores of the observation group were lower than those of the control group at 1 week,2 weeks and 4 weeks after operation(P<0.05).Conclusion:Mixed filling of bone cavity with CGF and allogeneic bone particles combined with biofilm covering can enhance the therapeutic effect of bone augmentation on odontogenic cyst,promote the recovery of affected teeth,reduce the incidence of postoperative infection,and inhibit scar formation after operation.
