针灸联合推拿对脑卒中后肩关节痛患者疼痛及关节功能的影响

Effect of Acupuncture Combined with Massage on Pain and Joint Function in Patients with Shoulder Arthralgia after Stroke

目的:分析针灸联合推拿对脑卒中后肩关节痛患者疼痛及关节功能的影响。方法:选取2020年3月—2022年10月泰安荣军医院收治的脑卒中后肩关节痛患者108例纳入研究,按电脑随机数字表法分为观察组54例,对照组54例。对照组采用推拿进行治疗,观察组在对照组基础上加用针灸,对比两组疼痛程度及关节功能。结果:治疗后,观察组总有效率高于对照组(P<0.05);治疗后,两组症状评分均较治疗前降低,且观察组症状评分均低于对照组(P<0.05);治疗后,两组视觉模拟评分法(VAS)、上肢功能障碍评定量表(DASH)评分均较治疗前降低,且观察组上述评分均低于对照组(P<0.05);治疗后,两组Fugl-Meyer运动功能评估法(FMA)上肢评分、改良Barthel指数(MBI)评分、日常生活能力(ADL)量表评分均较治疗前升高,且观察组上述评分均高于对照组(P<0.05)。结论:针灸联合推拿应用于脑卒中后肩关节痛治疗中,可缓解肩部关节疼痛,促进上肢功能恢复,提高生活自理能力。

Objective:To analyze the effect of acupuncture combined with massage on the pain and joint function of patients with shoulder arthralgia after stroke.Method:A total of 108 patients with shoulder joint pain after stroke admitted to Tai’an Rongjun Hospital from March 2020 to October 2022 were included in the study,and were divided into the observation group(54 cases)and the control group(54 cases)according to the method of computer random number table.The control group was treated with massage,and the observation group was treated with acupuncture on the basis of the control group.The pain degree and joint function of the two groups were compared.Result:After treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05);after treatment,the symptom scores of the two groups were lower than those before treatment,and the symptom scores of the observation group were lower than those of the control group(P<0.05);after treatment,the visual analogue scale(VAS)scores and disability of the arm,shoulder and hand(DASH)scores of the two groups were lower than those before treatment,and the scores of the observation group were lower than those of the control group(P<0.05);after treatment,the FMA upper limb scores,MBI scores and ADL scale scores of the two groups were higher than those before treatment,and the above scores in the observation group were higher than those in the control group(P<0.05).Conclusion:The application of acupuncture and massage in the treatment of shoulder arthralgia after stroke can relieve shoulder joint pain,promote the recovery of upper limbs function and improve selfcare ability.
