FTS-CNP模式的护理干预对胫骨平台骨折患者术后康复及VAS评分的影响

Effect of Nursing Intervention of FTS-CNP Model on Postoperative Rehabilitation and VAS Score of Patients with Tibial Plateau Fracture

目的:探讨快速康复外科(FTS)联合临床护理路径(CNP)对胫骨平台骨折患者术后康复及视觉模拟评分法(VAS)评分的影响。方法:选取2020年2月—2022年2月泉州市第一医院城东分院骨科收治的200例胫骨平台骨折患者,按照入院时间分为两组,各100例。对照组实施常规护理,观察组在对照组基础上实施FTS-CNP模式的护理干预。比较两组术后康复指标,采用VAS评价两组不同时间点的疼痛程度,并对两组Lysholm膝关节评分、并发症发生率、日常生活活动能力及护理满意度进行比较。结果:观察组术后康复指标均优于对照组(P<0.05)。与术前比较,两组术后3 d、出院时VAS评分均降低(P<0.05);与术后3 d比较,两组出院时VAS评分均降低,观察组术后3 d VAS评分低于对照组(P<0.05)。术后90 d两组Lysholm膝关节评分、Barthel指数评分均较术前升高,且观察组均高于对照组(P<0.05)。观察组术后并发症发生率(7.00%)较对照组(24.00%)低(P<0.05)。观察组护理总满意度(93.00%)较对照组(80.00%)高(P<0.05)。结论:FTS-CNP模式的护理干预应用在胫骨平台骨折患者中的效果较好,可减轻患者术后疼痛,促进康复。

Objective:To explore the effect of nursing intervention of fast track surgery(FTS)-clinical nursing pathway(CNP)model on postoperative rehabilitation and visual analogue scale(VAS)score of patients with tibial plateau fracture.Method:A total of 200 patients with tibial plateau fracture admitted to the Department of Orthopedics,Chengdong Branch of Quanzhou First Hospital from February 2020 to February 2022 were selected and divided into two groups according to the admission time,with 100 cases in each group.The control group received routine nursing,and the observation group received nursing intervention of FTS-CNP model based on the control group.Postoperative rehabilitation indexes were compared between the two groups.VAS was used to evaluate the pain degree of the two groups at different time points,and Lysholm knee joint score,incidence of complication,activity ability of daily living and nursing satisfaction were compared between the two groups.Result:Postoperative rehabilitation indexes of the observation group were better than those of the control group(P<0.05).Compared with before surgery,the VAS scores of both groups were decreased at 3 days after surgery and at discharge(P<0.05).Compared with 3 days after surgery,the VAS scores of both groups at discharge were lower,and the VAS score of the observation group at 3 days after surgery was lower than that of the control group(P<0.05).90 d after surgery,Lysholm knee joint scores and Barthel index scores were higher in both groups than those before surgery,and those in the observation group were higher than those in the control group(P<0.05).The incidence of postoperative complications in the observation group(7.00%)was lower than that in the control group(24.00%)(P<0.05).The total nursing satisfaction in observation group(93.00%)was higher than that in control group(80.00%)(P<0.05).Conclusion:The application of nursing intervention of FTS-CNP model in patients with tibial plateau fracture has a good effect,which can reduce postoperative pain and promote rehabilitation.
