72例肾综合征出血热流行病学及临床特征分析

Epidemiological and Clinical Characteristics of 72 Cases of Hemorrhagic Fever with Renal Syndrome

目的探究肾综合征出血热的流行病学及临床特征分析的,从而为临床疾病防控及诊疗提供科学的指导。方法回顾性分析我院2017年6月至2019年6月,经确诊收治的肾综合征出血热患者72例,对其人群分布、临床症状体征、实验室检查、危险因素、治疗费用及预后情况进行分析。结果72例患者中40~59岁年龄阶段所占比例最多44.44%,其中农民为主要群体占比80.56%;症状主要为轻型(34.77%)和中型(37.50%);临床症状及体征最为常见是发热(91.67%),其次是乏力(66.67%)、结膜水肿或充血(44.44%)、恶心呕吐(43.06%);临床表现主要为尿蛋白阳性97.22%、血小板减少93.06%、白细胞升高62.50%;在疾病暴露因素中占比最高为与鼠有接触史、与鼠排泄物有接触史,住院治疗平均时间为9.36 d、平均治疗费用为1.72万元。结论肾综合征出血热患者多数为轻型和中型,少数存在典型临床表现,如发热、尿蛋白、血小板减少等。应从相关暴露因素出发加强防治,强化疾病认识。

Objective To explore the epidemiological and clinical characteristics of hemorrhagic fever with renal syndrome(HFRS),so as to provide scientific guidance for the prevention and treatment of clinical diseases.Methods 72 patients with hemorrhagic fever of renal syndrome admitted and treated in our hospital from June 2017 to June 2019 were retrospectively analyzed,and their population distribution,clinical signs and symptoms,laboratory examination,risk factors,treatment cost and prognosis were analyzed.Results Among the 72 patients,the proportion between 40 and 59 years old was 44.44%,among which farmers accounted for 80.56%.Symptoms were mainly mild(34.77%)and moderate(37.50%).The most common clinical symptoms and signs were fever(91.67%),followed by fatigue(66.67%),conjunctival edema or congestion(44.44%),nausea and vomiting(43.06%).The main clinical manifestations were urinary protein positive 97.22%,thrombocytopenia 93.06%,leukocyte elevation 62.50%.The highest proportion of the disease exposure factors was the history of contact with mice and the history of contact with mouse excreta.The average hospitalization time was 9.36 days and the average treatment cost was 17200 yuan.Conclusion The majority of patients with hemorrhagic fever of renal syndrome are light and medium,and a few have typical clinical manifestations,such as fever,urinary protein,thrombocytopenia,etc.Prevention and treatment should be strengthened from relevant exposure factors to strengthen disease understanding.
