全身免疫-炎症指数与糖尿病视网膜病变相关性分析

Correlation Between Systemic Immune Inflammation Index and Diabetes Retinopathy

目的 分析全身免疫-炎症指数(systemic immune inflammatory index, SII)与糖尿病视网膜病变(diabetes retinopathy, DR)的相关性。方法 选取2021-01/2022-12月作者单位内分泌科收治入院的668例2型糖尿病(type 2 diabetes mellitus, T2DM)患者为研究对象。根据眼底检查结果将患者分为单纯T2DM组(n=505)及T2DM合并DR组(n=163)。检测所有患者空腹血常规,获得中性粒细胞计数、淋巴细胞计数及血小板计数,计算SII值,分析SII值与DR的相关性。结果 T2DM合并DR组病程、糖化血红蛋白(glycosylated hemoglobin, HbA1c)、糖化血红蛋白变异指数(haemoglobin glycation index, HGI)比例及SII均高于单纯T2DM组,肾小球滤过率(estimated glomerular filtration rate, eGFR)水平低于单纯T2DM组,差异具有统计学意义(P均<0.05)。二元Logistic回归分析结果显示,糖尿病长病程、高HbA1c、高HGI及高SII是T2DM患者发生DR的危险因素,高eGFR则是T2DM患者发生DR的保护因素。结论 SII在DR患者中明显升高,与DR的发生呈正相关,临床上可作为预测T2DM患者发生DR风险的新型实验室指标。

Objective To analyze the correlation between systemic immune inflammatory index(SII)and diabetes retinopathy(DR).Methods A total of 668 patients with type 2 diabetes mellitus(T2DM)admitted to the endocrine department of author's hospital from January 2021 to December 2022 were selected,patients were divided into pure T2DM group(n=505)and T2DM combined DR group(n=163)according to results of fundus examination.The fasting blood routine test was performed among all patients to obtain neutrophil counts,lymphocyte counts and platelet counts,SII value was calculated and the correlation between SII value and DR was analyzed.Results The course of disease,glycosylated hemoglobin(HbArc),haemoglobin glycation index(HGI)ratio and SII index in T2DM combined DR group were all higher than those in the pure T2DM group,and the estimated glomerular filtration rate(eGFR)level was lower than that in the pure T2DM group,and the difference was statistically significance(all P<0.05).The binary Logistic regression analysis showed that the risk factors for DR in T2DM patients were long course of diabetes mellitus,high HbAr c,high HGI,and high SII,while high eGFR was the protective factor for DR in T2DM patients.Conclusion The SII significantly increases in DR patients and was positively correlated with the occurrence of DR,which can be used as a new laboratory indicator to predict the risk of DR in T2DM patients in clinical practice.
