气管隆凸肿瘤的外科治疗进展

Surgical advances in the carinal resection and reconstruction for tracheal tumor

摘要 隆凸肿瘤的切除与重建因复杂的麻醉管理、高难度的吻合技术、高发生率的术后并发症和手术死亡率, 一直是对胸外科医生的能力最具有挑战性的手术。近年来随着外科技术和麻醉管理的进步, 胸腔镜微创技术和达芬奇机器人用于气管隆凸肿瘤的切除与重建, 提高了该类患者手术的成功率和术后生活质量, 降低了术后并发症和手术死亡率。本文拟根据相关研究, 总结隆凸肿瘤的切除与重建的新进展, 重点介绍个体化隆凸肿瘤切除与重建和微创隆凸肿瘤切除与重建的主流治疗观点。 Surgical treatment of tracheal tumors with carinal resection and reconstruction has always posed significant challenges to thoracic surgeons due to complex anesthesia management,highly demanding anastomosis techniques,high incidence of postoperative complications,and surgical mortality rate.In recent years,with the improvement in surgical techniques and anesthesia management,the introduction of the video-assisted thoracoscopic surgery(VATS)and robotic-assisted thoracic surgery(RATS)has enhanced the success rate of surgical procedures and postoperative quality of life for patients with tracheal tumors,while reducing postoperative complications and surgical mortality rate.This article provides a comprehensive overview of the most recent surgical advancements in the field of tracheal tumors,specifically focusing on carinal resection and reconstruction,highlighting the mainstream treatment perspectives of the personalized approaches to resection and reconstruction,along with the utilization of minimally invasive techniques.
