反馈式抗阻吸气肌训练对食管癌术后患者的影响

摘要 目的探究反馈式抗阻吸气肌训练对食管癌术后患者吸气肌力量及呼吸功能的影响。方法本研究为前瞻性队列研究,选取2020年3月至2022年9月河南科技大学第一附属医院肿瘤内科收治的89例食管癌术后患者,男57例,女32例,年龄(59.86±7.26)岁,年龄范围为51~69岁。采用随机数表法将患者随机分为常规呼吸功能锻炼组(n=44)与反馈式抗阻吸气肌训练组(n=45)。常规呼吸功能锻炼组给予常规呼吸功能锻炼,反馈式抗阻吸气肌训练组联合反馈式抗阻吸气肌训练。比较两组患者的吸气肌力量、呼吸困难程度、呼吸功能及呼吸系统并发症发生率。结果反馈式抗阻吸气肌训练组患者的最大呼气压、最大吸气压水平[(7.34±0.66)kPa、(7.75±0.76)kPa]高于常规呼吸功能锻炼组[(6.38±0.57)kPa、(6.42±0.55)kPa],差异有统计学意义(P<0.05)。反馈式抗阻吸气肌训练组改良borg呼吸困难评分[(1.62±0.46)分]低于常规呼吸功能锻炼组[(3.16±0.57)分],差异有统计学意义(P<0.05)。反馈式抗阻吸气肌训练组第1秒用力呼气容积占预计值百分比(FEV1%)、用力肺活量(FVC)、第1秒用力呼气容积/用力肺活量(FEV1/FVC)水平[(77.52±2.76)%、(2.93±0.36)L、(76.34±2.95)%]高于常规呼吸功能锻炼组[(73.26±2.97)%、(2.54±0.45)L、(72.57±2.86)%],差异有统计学意义(P<0.05)。结论反馈式抗阻吸气肌训练能够有效增强食管癌术后患者吸气肌力量,降低呼吸困难程度,改善呼吸功能。 Objective To explore the effects of feedback anti-obstructive inspiratory muscle training on inspiratory muscle strength and respiratory function in patients with esophageal cancer after surgery.Methods This study was a prospective cohort study,a total of 89 postoperative patients with esophageal cancer,including 57 males and 32 females,aged(59.86±7.26)years old,the age was ranging from 51 to 69 years old,were selected from the department of Oncology of the First Affiliated Hospital of Henan University of Science and Technology from March 2020 to September 2022.Patients were randomly divided into routine respiratory function exercise group(n=44)and feedback anti-obstruction inspiratory muscle training group(n=45)by using random number table method consistent with the text.The routine respiratory function exercise group was given routine respiratory function exercise,and the feedback anti-obstruction inspiratory muscle training group was combined with feedback anti-obstruction inspiratory muscle training.The inspiratory muscle strength,dyspnea degree,respiratory function and incidence of respiratory complications were compared between the two groups.Results The maximum expiratory pressure and maximum inspiratory pressure levels in feedback anti-obstruction inspiratory muscle training group[(7.34±0.66)kPa,(7.75±0.76)kPa]were higher than those in routine respiratory function exercise group[(6.38±0.57)kPa,(6.42±0.55)kPa],and the differences were statistically significant(P<0.05).The borg dyspnea score of feedback anti-obstruction inspiratory muscle training group[(1.62±0.46)points]was lower than that of routine respiratory function exercise group[(3.16±0.57)points],and the difference was statistically significant(P<0.05).The levels of forced expiratory volume in one second%estimated value(FEV1%),forced vital capacity(FVC),forced expiratory volume in 1 second/forced vital capacity(FEV1/FVC)in feedback anti-obstruction inspiratory muscle training group[(77.52±2.76)%,(2.93±0.36)L,(76.34±2.95)%]were higher than those in routine respiratory function exercise group[(73.26±2.97)%,(2.54±0.45)L,(72.57±2.86)%],and the differences were statistically significant(P<0.05).Conclusions Feedback anti-obstructive inspiratory muscle training can effectively enhance inspiratory muscle strength,reduce dyspnea and improve respiratory function in patients with esophageal cancer after surgery.
