难治性勃起功能障碍中西医结合防治策略述评

Comment on prevention and treatment strategy of integrated traditional Chinese and Western medicine in refractory erectile dysfunction

难治性勃起功能障碍(RED)属于勃起功能障碍(ED)的特殊类型,一般认为RED经口服5型磷酸二酯酶抑制剂(PDE5I)治疗效果不佳甚或无效。文章旨在立足于中西医结合视角,初步提出RED的概念和范畴,并以心理性ED、糖尿病性ED、海绵体神经损伤性ED、动脉粥样硬化性ED、高血压性ED、性腺功能减退所致ED为例,结合发病机制、中医病机和研究现况等予以探讨,继而浅析RED中西医结合防治策略的关键点及其内涵,认为中西医结合治疗RED具有合理性、潜力和优势,进一步探索优化RED的中西医结合防治策略当是具有价值的研究方向。

Refractory erectile dysfunction(RED)is one special type of erectile dysfunction(ED).It is generally considered that the effect of oral 5-phosphodiesterase inhibitor(PDE5I)on RED is poor or even ineffective.Based on the perspective of integration of traditional Chinese medicine(TCM)and western medicine,the concept and category of RED were tentatively put forward.Several common RED(psychogenic erectile dysfunction,diabetic ED,cavernous nerve injuryinduced ED,atherosclerotic ED,hypertension combined ED,hypogonadism-induced ED)were taken as examples and introduced combined with pathogenesis,TCM pathogenesis and research status.Then the key points and connotation of the prevention and treatment strategy of integrated TCM and western medicine for RED were proposed.It was believed that treatment of red with integrated TCM and western medicine had rationality,potential and advantages.Further exploring and optimizing the prevention and treatment strategy of integrated TCM and western medicine in RED should be a valuable research direction.
