摘要
目的:探讨甲泼尼龙联合乙酰半胱氨酸溶液对急性呼吸窘迫综合征通气患者血气指标及肺容积的影响。方法:选取医院2020年12月—2022年12月100例急性呼吸窘迫综合征通气患者,根据随机数字表法分为对照组和观察组,各50例。对照组给予吸入用乙酰半胱氨酸溶液,观察组在此基础上给予注射用甲泼尼龙琥珀酸钠。比较两组血气指标[氧分压(PaO_(2))、吸入氧浓度分数(FiO_(2))]、肺容积指标[功能残气量(FRC)、肺总量(TLC)]、炎症因子[干扰素-γ(IFN-γ)、白介素-4(IL-4)、白介素-10(IL-10)]、机械通气时间、住院时间、病死率及不良反应发生率。结果:观察组PaO_(2)、PaO_(2)/FiO_(2)、FRC、TLC高于对照组,FiO_(2)、IFN-γ、IL-4、IL-10低于对照组(P<0.05);观察组机械通气时间、住院时间短于对照组,病死率低于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:甲泼尼龙联合乙酰半胱氨酸溶液可改善急性呼吸窘迫综合征通气患者的血气指标和肺容积,降低炎症因子水平,缩短机械通气和住院时间,降低病死率,且具有安全性。
Objective:To investigate the effect of methylprednisolone combined with acetylcysteine solution on blood gas index and lung volume in patients with acute respiratory distress syndrome.Methods:A total of 100 patients with acute respiratory distress syndrome ventilation in our hospital from December 2020 to December 2022 were selected and divided into control group and observation group according to random number table method,with 50 patients in each group.The control group was given acetylcysteine solution for inhalation,and the observation group was given methylprednisolone sodium succinate for injection on this basis.The blood gas indexes[oxygen partial pressure(PaO_(2)),fraction of inspired oxygen(FiO_(2))],lung volume indexes[functional residual capacity(FRC),total lung capacity(TLC)],inflammatory factors[interferon-γ(IFN-γ),interleukin-4(IL-4),interleukin-10(IL-10)],mechanical ventilation time,hospitalization time,mortality and incidence of adverse reactions were compared between the two groups.Results:PaO_(2),PaO_(2)/FiO_(2),FRC and TLC in the observation group were higher than those in the control group,while FiO_(2),IFN-γ,IL-4 and IL-10 were lower than those in the control group(P<0.05).The mechanical ventilation time and hospitalization time of the observation group were shorter than those of the control group,and the mortality rate was lower than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Methylprednisolone combined with acetylcysteine solution can improve the blood gas index and lung volume of patients with acute respiratory distress syndrome ventilation,reduce the level of inflammatory factors,shorten the time of mechanical ventilation and hospitalization,reduce the mortality rate,and is safe.
