摘要
目的:探讨低分子右旋糖酐联合呋塞米治疗儿童肾病综合征水肿程度的临床效果。方法:选取2020年1月—2022年1月商丘市第一人民医院儿科收治的92例肾病综合征患儿为研究对象,按照随机数字表法分为对照组、观察组,各46例。对照组予以呋塞米治疗,观察组在对照组基础上联合低分子右旋糖酐治疗,比较两组临床疗效、尿生化指标、炎症因子、免疫功能指标、治疗安全性等。结果:观察组总有效率为91.30%,高于对照组的71.74%(P<0.05);观察组24hpro、BUN、TC低于对照组,ALB高于对照组(P<0.05);观察组血清AQP3、PPAR-α、KIM-1水平均低于对照组(P<0.05);观察组IgA、IgG、IgM高于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:低分子右旋糖酐联合呋塞米治疗儿童肾病综合征水肿具有良好应用效果,可改善患儿的尿生化指水平,抑制PPAR-α、KIM-1、AQP3表达,提升患儿的免疫功能,且联合治疗不增加不良反应发生概率,安全性较高。
Objective:To investigate the clinical effect of low molecular dextran combined with furosemide in the treatment of edema in children with nephrotic syndrome.Methods:A total of 92 children with nephrotic syndrome admitted to the pediatrics department of Shangqiu First People’s Hospital from January 2020 to January 2022 were selected as the research objects.According to the random number table method,they were divided into control group and observation group,with 46 cases in each group.The control group was treated with furosemide,and the observation group was treated with low molecular dextran on the basis of the control group.The clinical efficacy,urine biochemical indexes,inflammatory factors,immune function indexes and treatment safety of the two groups were compared.Results:The total effective rate of observation group was 91.30%,which was higher than that of control group 71.74%(P<0.05).The 24HPro,BUN and TC of the observation group were lower than those of the control group,and the ALB of the observation group was higher than that of the control group(P<0.05).The serum levels of AQP3,PPAR-αand KIM-1 in the observation group were lower than those in the control group(P<0.05).IgA,IgG and IgM of the observation group were higher than those of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Low molecular dextrose combined with furosemide has a good application effect in the treatment of nephrotic syndrome edema in children,which can improve the urine biochemical finger level,inhibit the expression of PPAR-α,KIM-1 and AQP3,and improve the immune function of children.The combined treatment does not increase the incidence of adverse reactions,and has high safety.
