摘要 糖尿病肾病(DN)是一种免疫参与的炎症性疾病,微炎症状态贯穿DN发病始终。细胞焦亡是一种促炎的程序性细胞死亡,主要包括半胱氨酸蛋白酶-1(caspase-1)依赖的经典焦亡途径和caspase-4、5和11依赖的非经典焦亡途径。异常的细胞焦亡及炎症反应损伤肾脏固有细胞,导致肾间质纤维化,并加速DN的进展。本文综述了细胞焦亡在DN发生发展中的作用及分子调控机制,以期为临床诊疗DN提供新思路。 Diabetic nephropathy(DN)is a chronic immune-mediated inflammatory disorder characterized by persistent microinflammation throughout its pathogenesis.A significant contributor to this inflammatory response is pyroptosis,a pro-inflammatory form of programmed cell death.Pyroptosis encompasses two main pathways:the canonical caspase-1 inflammasome pathway and the noncanonical caspase-4/5/11 inflammasome pathway.Dysregulation of pyroptosis and the ensuing inflammatory reactions inflict damage upon renal intrinsic cells,ultimately resulting in renal interstitial fibrosis and hastening the progression of DN.This comprehensive article provides a systematic review of the involvement of pyroptosis in the pathogenesis and development of DN,shedding light on the molecular mechanisms that govern this intricate process.By elucidating these mechanisms,novel insights can be gained to enhance the clinical diagnosis and treatment of DN,paving the way for more effective therapeutic strategies.
