The Relationship Between Palliative Care Knowledge Status and Perceived Self-competence in Emergency Care Nurses in Hubei Province

目的:探讨湖北省急诊科护士安宁疗护知识状况与安宁疗护自我感知能力的关系,为急诊科护士安宁疗护自我感知能力的机制研究和制定相应的培训方案提供参考。方法:选取湖北省22家三级医院415名急诊科护士为研究对象,使用一般资料问卷、中文版缓和医疗照护自我能力量表、安宁疗护知识问卷、中文版佛罗梅尔特临终关怀态度量表、中文版美国重症护理协会健康工作环境评估量表为调查工具,构建结构方程模型分析安宁疗护知识状况和安宁疗护自我感知能力间的关系。结果:湖北省急诊科护士安宁疗护自我感知能力与安宁疗护知识(r=0.189,P<0.01)、安宁疗护态度(r=0.220,P<0.01)、健康工作环境(r=0.373,P<0.01)均呈正相关。结构方程模型结果显示,安宁疗护知识仅通过安宁疗护态度和健康工作环境的链式中介作用影响安宁疗护自我感知能力,链式中介效应值为0.127。结论:急诊科护士安宁疗护知识状况与安宁疗护自我感知能力呈正相关。安宁疗护态度和健康工作环境在安宁疗护知识和安宁疗护自我感知能力间发挥链式中介作用,提示在制定相应干预措施时需要重点考虑环境因素和态度水平。

Objective:To investigate the relationship between palliative care knowledge status and perceived self-competence in palliative care among emergency care nurses in Hubei Province,so as to provide a reference for understanding the mechanism of perceived self-competence in palliative care among emergency care nurses and the development of relevant training programs.Methods:A total of 415 emergency care nurses from 22 tertiary hospitals in Hubei Province were selected using convenience sampling.The study utilized a structured questionnaire consisting of a general information questionnaire,the palliative care nursing self-competence scale,the palliative care knowledge quiz for nurses,the Frommelt attitudes toward care of the dying scale form B,and the AACN healthy work environment assessment tool.A structural equation model was constructed to analyze the relationships among palliative care knowledge status,attitude towards palliative care,healthy work environment,and perceived self-competence in palliative care.Results:Perceived self-competence in palliative care among emergency care nurses in Hubei province was positively correlated with palliative care knowledge(r=0.189,P<0.01),attitude towards palliative care(r=0.220,P<0.01),and healthy work environment(r=0.373,P<0.01).The structural equation model results indicated that palliative care knowledge influenced perceived self-competence in palliative care through the chain mediating effect of attitude towards palliative care and healthy work environment,with a chain mediating effect value of 0.127.Conclusions:Palliative care knowledge status among emergency care nurses is positively associated with perceived self-competence in palliative care.Attitude towards palliative care and healthy work environment play a chain mediating role between palliative care knowledge and perceived self-competence in palliative care.These findings highlight the importance of considering environmental factors and attitude levels when developing relevant intervention measures.
