摘要 目的探讨上呼吸道感染时干扰素(interferon,IFN)-α和IFN-γ对24~72h血液中性粒细胞的影响。方法选取2020年1月至2022年8月于杭州市儿童医院诊治的上呼吸道感染患儿92例,根据24~72h内中性粒细胞水平将其分为ANC组(中性粒细胞<1000×10^(6)/L,49例)和NT组(中性粒细胞≥1000×10^(6)/L,43例)。收集患儿的临床资料,采用多因素Logistic回归分析影响患儿中性粒细胞的危险因素,对最终筛选出的因素进行受试者操作特征曲线(receiver operating characteristic curve,ROC曲线)分析。结果NT组患儿的年龄大于ANC组,IFN-α、IFN-γ水平低于ANC组(P<0.05)。多因素Logistic回归分析结果显示IFN-α和IFN-γ升高均是24~72h血液中性粒细胞减少的独立危险因素(P<0.05)。ROC曲线显示,IFN-α、IFN-γ预测中性粒细胞减少的曲线下面积分别为0.734和0.719,两者均有较高的预测价值。结论IFN-α和IFN-γ可预测上呼吸道感染患儿的中性粒细胞水平,且均是患儿中性粒细胞减少的独立危险因素。 Objective To investigate the effects of interferon(IFN)-αand IFN-γon blood neutrophils during 24-72 hours of upper respiratory tract infection.Methods Ninety-two children with upper respiratory tract infection diagnosed and treated in Hangzhou Children’s Hospital from January 2020 to August 2022 were selected and divided into ANC group(49 cases with neutrophils<1000×10^(6)/L)and NT group(43 cases with neutrophils≥1000×10^(6)/L)according to the levels of neutrophils within 24-72 hours.The clinical data of the children were collected,and the risk factors affecting neutrophils in the children were analyzed by multivariate Logistic regression,and the receiver operating characteristic(ROC)curve was analyzed for the factors finally selected.Results The age of children in NT group was higher than that in ANC group,and the levels of IFN-αand IFN-γwere lower than those in ANC group(P<0.05).Multivariate Logistic regression analysis showed that IFN-αand IFN-γwere independent risk factors for blood neutropenia within 24-72 hours(P<0.05).ROC curve showed that area under the curve of IFN-αand IFN-γpredicted neutropenia were 0.734 and 0.719,respectively,both of which had high predictive value.Conclusion IFN-αand IFN-γpredict the level of neutrophils in blood of children with upper respiratory tract infection,and are independent risk factors for neutropenia in children.
