扩张型心肌病患者血清铁蛋白、BNP、NLR水平与左心室重构的关系

Correlation of levels of serum ferritin,BNP and NLR with left ventricular remodeling in patients with dilated cardiomyopathy

目的探讨扩张型心肌病患者血清铁蛋白、B型脑钠肽(BNP)、中性粒细胞/淋巴细胞(NLR)水平与左心室重构的关系。方法抽取2020年3月至2022年3月鹤壁煤业(集团)有限责任公司总医院收治的103例扩张型心肌病患者作为观察组,另抽取102例同期健康体检者作为对照组。比较对照组与观察组不同心功能等级患者铁蛋白、BNP、NLR和心功能指标[左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左心室质量指数(LVMI)]。分析扩张型心肌病患者血清铁蛋白、BNP、NLR与LVMI的相关性。结果观察组血清铁蛋白、BNP、NLR、LVEF、LVEDD、LVMI水平与对照组比较,差异有统计学意义(P<0.05);观察组不同心功能等级者血清铁蛋白、LVMF低于对照组,BNP、NLR、LVEDD、LVMI水平高于对照组(P<0.05);随着观察组患者心功能等级的升高,其铁蛋白逐渐降低,BNP、NLR、LVEDD、LVMI水平逐渐升高(P<0.05)。相关性分析结果显示,铁蛋白与LVMI呈负相关(r=-0.42,P<0.05),BNP、NLR与LVMI呈正相关(r=0.48、0.44,P均<0.05)。结论铁蛋白与扩张型心肌病患者左心室重构呈负相关,BNP和NLR与左心室重构呈正相关。

ObjectiveTo investigate the correlation of the levels of ferritin,B-type natriuretic peptide(BNP)and neutrophil-to-lymphocyte ratio(NLR)with left ventricular remodeling in patients with dilated cardiomyopathy.MethodsA total of 103 patients with dilated cardiomyopathy admitted to the General Hospital of Hebi Coal Industry Group Co.,Ltd.from March 2020 to March 2022 were selected as the observation group,and 102 healthy patients who underwent physical examination during the same period were selected as the control group.The ferritin,BNP,NLR and cardiac function indexes,including left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),left ventricular mass index(LVMI),were compared between patients in the control group and patients with different cardiac function grades in the observation group.The correlation of levels of ferritin,BNP and NLR with LVMI in patients with dilated cardiomyopathy was analyzed.ResultsThere were statistically significant differences in levels of serum ferritin,BNP,NLR,LVEF,LVEDD and LVMI between the control group and the observation group(P<0.05).The levels of serum ferritin and LVMF in patients with different cardiac function grades in the observation group were lower than those in the control group,however,the levels of BNP,NLR,LVEDD and LVMI were higher than those in the control group(P<0.05).With the increase of cardiac function grade in the observation group,ferritin level gradually decreased,and the levels of BNP,NLR,LVEDD and LVMI gradually increased(P<0.05).The correlation analysis showed that ferritin was negatively correlated with LVMI(r=-0.42,P<0.05),moreover,BNP and NLR were positively correlated with LVMI(r=0.48,0.44;all P<0.05).ConclusionsSerum ferritin is negatively correlated with left ventricular remodeling,while BNP and NLR are positively correlated with left ventricular remodeling in patients with dilated cardiomyopathy.
