摘要
目的分析核共振成像(MRI)及电子断层扫描(CT)在膝半月板损伤与关节软骨损伤临床诊断中的应用价值。方法随机选取2019年1月—2022年9月本院诊治的118例膝半月板损伤与关节软骨损伤患者进行研究。所有患者入院后均接受MRI、CT和关节镜检查,以关节镜诊断结果为金标准,分析MRI、CT及联合检查的临床诊断价值。结果MRI对膝半月板损伤的检出率为93.94%,高于CT检查的81.82%(χ^(2)=6.828,P=0.014);MRI对关节软骨损伤总检出率为94.68%,高于CT检查的82.98%(χ^(2)=6.486,P=0.011)。MRI诊断膝半月板损伤的灵敏度、准确率为91.92%、91.53%,均高于CT检查的79.80%、81.36%(χ^(2)=5.990、5.206,P均<0.05);MRI诊断膝半月板损伤的结果与金标准的一致性高于CT检查(Kappa值=0.722 vs 0.501);MRI诊断关节软骨损伤的灵敏度为92.55%,准确率为92.37%,均高于CT的79.79%、81.36%(χ^(2)=6.427、6.276,P均<0.05);MRI检查关节软骨损伤的结果与金标准的一致性高于CT检查(Kappa值=0.782 vs 0.539)。结论MRI在诊断膝半月板损伤与关节软骨损伤中的应用价值高于CT。
Objective To analyze the application value of magnetic resonance imaging(MRI)and computerized tomography(CT)in the clinical diagnosis of knee meniscal injury and articular cartilage injury.Methods A total of 118 patients with knee meniscal injury and articular cartilage injury treated by our hospital from January 2019 to September 2022 were selected at random for study.All the patients underwent MRI,CT and arthroscopy after admission.The clinical diagnostic values of MRI,CT and combined examinations were analyzed with arthroscopic diagnosis results as the gold standard.Results The detection rate of knee meniscal injury by MRI was 93.94%,higher than that of CT:81.82%(χ^(2)=6.828,P=0.014).The overall detection rate of articular cartilage injury by MRI was 94.68%,higher than that of CT:82.98%(χ^(2)=6.486,P=0.011).The sensitivity and accuracy of MRI in the diagnosis of knee meniscus injury were 91.92%and 91.53%respectively,which were higher than 79.80%and 81.36%of CT(χ^(2)=5.990,5.206,both P<0.05).The consistency of MRI diagnosis of knee meniscus injury with the gold standard was higher than that of CT(Kappa value=0.722 vs 0.501).The sensitivity and accuracy of MRI in the diagnosis of articular cartilage injury were 92.55%and 92.37%respectively,which were higher than 79.79%and 81.36%of CT(χ^(2)=6.427,6.276,both P<0.05).The consistency between MRI and the gold standard was higher than that of CT(Kappa value=0.782 vs 0.539).Conclusion The application values of MRI in the diagnosis of knee meniscal injury and articular cartilage injury are higher than those of CT.
