川崎病患儿血清总IgE、富亮氨酸α-2糖蛋白升高与冠状动脉病变的发生率、预后的相关性及影响因素分析

摘要 目的探究川崎病患儿血清总免疫球蛋白IgE(Immunoglobulin,IgE)、富亮氨酸α-2糖蛋白1(Leucine-richα-2 Glycoprotein-1,LRG1)升高与冠状动脉病变的发生率及预后相关性,并就川崎病患儿出现冠状动脉病变的危险因素开展分析。方法选择2020年1月—2022年12月本院儿童医院确诊为川崎病的127例患儿为研究对象,根据切面超声心动图对冠状动脉检测结果分为冠脉病变组(n=60)和无冠脉病变组(n=67)两组。比较两组患儿IgE、LRG1水平差异,对入组患者开展积极的治疗后,比较治疗前后入组患儿IgE、LRG1水平变化,最后分析导致川崎病患儿出现冠状动脉病变的危险因素。结果冠脉病变组患儿血清IgE、LRG1水平均明显高于无冠脉病变组患儿,差异具有统计学意义(P<0.05);治疗后入组川崎病患儿的血清IgE、LRG1水平均出现了明显的降低,差异具有统计学意义(P<0.05);通过非条件多因素Logistic回归分析显示,发热持续时间、CRP水平过高、IgE水平过高和LRG1水平过高是导致川崎病患儿出现冠状动脉损伤的独立危险因素,差异具有统计学意义(P<0.05)。结论川崎病患儿出现冠状动脉损伤几率较高,患儿血清IgE、LRG1水平会出现异常升高;发热持续时间、CRP水平过高、IgE水平过高和LRG1水平过高是导致川崎病患儿出现冠状动脉损伤的独立危险因素,对出现上述危险因素的川崎病患儿,建议加强干预,以改善其预后。 Objective To research the correlation between total serum Immunoglobulin E(IgE)and elevated leucine-richα-2 glycoprotein-1(LRG1)and incidence of coronary artery disease and prognosis in children with Kawasaki disease,and analyze the risk factors of coronary artery disease in children with Kawasaki disease.Methods 127 children with Kawasaki disease diagnosed in children's hospital Suzhou municipal hospital,from January 2020 to December 2022 were selected as the study subjects.According to the results of coronary artery examination by sectional echocardiography,they were divided into coronary artery disease group(n=60)and non-coronary artery disease group(n=67).The differences of IgE and LRG1 levels between the two groups were compared.After active treatment,the changes of IgE and LRG1 levels in the enrolled children before and after treatment were compared.Finally,the risk factors of coronary artery disease in children with Kawasaki disease were analyzed.Results The levels of serum IgE and LRG1 in patients with coronary artery disease were significantly higher than those in patients without coronary artery disease(P<0.05).After treatment,the levels of serum IgE and LRG1 in children with Kawasaki disease were significantly decreased(P<0.05).The unconditional multivariate logistic regression analysis showed that the duration of fever,the high level of CRP,the high level of IgE and the high level of LRG1 were the independent risk factors for coronary artery injury in children with Kawasaki disease,and the differences were statistically significant(P<0.05).Conclusions The incidence of coronary artery injury in children with Kawasaki disease is high,and the levels of serum IgE and LRG1 in children with Kawasaki disease will increase abnormally.Unconditional multivariate logistic regression analysis shows that the duration of fever,the high level of CRP,the high level of IgE and the high level of LRG1 are independent risk factors for coronary artery injury in children with Kawasaki disease.For children with Kawasaki disease with the above risk factors,it is recommended to strengthen intervention to improve their prognosis.
