普通电切镜和等离子电切镜在前列腺增生中的效果分析

Analysis of the Effect of Ordinary Electrosurgical Resection and Plasma Electrosurgical Resection in Prostatic Hyperplasia

目的评估普通电切镜、等离子电切镜应用在前列腺增生(prostatic hyperplasia,PH)手术治疗中的效果及安全性。方法纳入2021年7月—2023年1月上海中医药大学附属第七人民医院泌尿外科的76例PH患者,参照随机数字表法分为对照组(纳入38例,行普通电切镜手术)、观察组(纳入38例,行等离子电切镜手术)。评价两组临床有效率、围手术期参数、尿流动力学、国际前列腺症状评分(international prostate symptom score,IPSS)、射精功能评分(ejaculatory function score,CIPE-5)、生活质量综合评定问卷(comprehensive quality of life assessment questionnaire-74,GQOLI-74)、并发症。结果(1)观察组临床总有效率高于对照组,手术用时、住院时间、尿管留置时间短于对照组,手术出血量及并发症总发生率低于对照组(P<0.05);(2)术前,两组尿流动力学、IPSS、CIPE-5、GQOLI-74比较,差异无统计学意义(P>0.05);术后,观察组尿流动力学、IPSS、CIPE-5、GQOLI-74更佳(P<0.05)。结论在PH手术治疗中采用等离子电切镜手术,有助于增强疗效,优化围手术期参数,减少并发症及性功能障碍,改善排尿功能与生活质量。

Objective To evaluate the efficacy and safety of ordinary electrosurgical resection and plasma electrosurgical resection in the surgical treatment of prostatic hyperplasia(PH).Methods A total of 76 PH patients in the department of urology,the Seventh People's Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from July 2021 to January 2023 were included.The control group(38 patients were included,and ordinary electrosurgical resection were performed)and the observation group(38 patients were included,and plasma electrosurgical resection were performed)were divided according to the random number table.The clinical efficiency,perioperative parameters,urodynamics international prostate symptom score(IPSS),ejaculatory function score(CIPE-5),comprehensive quality of life assessment questionnaire-74(GQOLI-74),and comorbidities.Results(1)The total clinical effective rate of the observation group was higher than that of the control group,the operation time,hospital stay and urinary catheter retention time were shorter than that of the control group,and the total incidence of surgical bleeding and complications were lower than that of the control group(P<0.05).(2)Before surgery,there was no difference in urodynamics,IPSS,CIPE-5,and GQOLI-74 between groups(P>0.05).After surgery,the observation group showed better urodynamics,IPSS,CIPE-5,and GQOLI-74(P<0.05).Conclusion The use of plasma resectoscope in PH surgery is helpful to enhance the efficacy,optimize perioperative parameters,reduce complications and sexual dysfunction,improve urination function and quality of life.
