老年阑尾炎患者应用腹腔镜阑尾切除术与传统术式的临床效果对比研究

Comparative Study on the Clinical Effect of Laparoscopic Appendectomy and Traditional Appendectomy in Elderly Patients with Appendicitis

目的:探讨传统手术及腹腔镜治疗老年阑尾炎的临床效果。方法:选取2021年1月—2023年1月和县人民医院普通外科收治的老年阑尾炎患者60例为观察对象,根据随机数字表法分为对照组与观察组,各30例。对照组采取传统手术治疗,观察组采取腹腔镜手术治疗。比较两组患者手术指标、疼痛及并发症发生情况。结果:观察组患者手术时间、胃肠功能恢复时间、排气时间、下床时间及住院时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.001);术后4、12、24、48h,观察组患者疼痛评分均低于对照组,差异有统计学意义(P<0.05);观察组患者并发症发生率低于对照组,差异有统计学意义(P=0.030)。结论:腹腔镜应用于老年阑尾炎患者中效果显著,可改善手术指标,降低疼痛程度及并发症发生率。

Objective:To explore the clinical effect of traditional operation and laparoscopic treatment of senile appendicitis.Methods:Sixty elderly patients with appendicitis admitted to the Department of General Surgery,Hexian People's Hospital from January 2021 to January 2023 were selected as the observation objects,and were divided into control group and observation group according to random number table method,with thirty cases in each group.The control group was treated with traditional surgery,and the observation group was treated with laparoscopic surgery.The surgical indexes,pain and complications were compared between the two groups.Results:The operation time,gastrointestinal function recovery time,exhaust time,getting out of bed time and hospitalization time in observation group were shorter than those in control group,and the intraoperative blood loss was less than that in control group,the difference was statistically significant(P<0.001);At 4,12,24 and 48 h after operation,the pain scores of observation group were lower than those of control group,the difference was statistically significant(P<0.05);The incidence of complications in observation group was lower than that in control group,and the difference was statistically significant(P=0.030).Conclusion:The application of laparoscopy in elderly patients with appendicitis has a remarkable effect,which can improve the surgical indicators,reduce the degree of pain and the incidence of complications.
