常规血脂检验在预测糖尿病合并冠心病风险中的应用

Application of Routine Lipid Tests in Predicting the Risk of Diabetes Mellitus Combined with Coronary Heart Disease

目的:分析常规血脂检验在预测糖尿病合并冠心病发生风险中的应用效果。方法:选取2020年1月—2023年3月遵义市红花岗区人民医院行血脂检验的冠心病合并糖尿病受检者100例为研究对象,根据Gennisi积分对患者进行分组,分别为低积分组(n=40)、中积分组(n=35),高积分组(n=25)。患者均进行血脂检测,记录血脂检测结果及各指标比值,与Gennisi积分做相关性分析。结果:三组总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)水平比较,差异有统计学意义(P<0.05);三组TC/高密度脂蛋白胆固醇(HDL-C)、LDL-C/HDL-C比较,差异有统计学意义(P<0.05);在控制影响因素空腹血糖和HbA1c后,发现TC/HDL-C、LDL-C/HDL-C水平与Gennisi积分呈正相关(P<0.001)。结论:除常规血脂指标外,TC/HDL-C、LDL-C/HDL-C水平对糖尿病患者冠心病发生风险也具有较高的预测价值,可为临床诊断提供参考依据。

Objective:To analyze the application effect of routine lipid test in predicting the risk of coronary heart disease in patients with diabetes mellitus combined with coronary heart disease.Methods:One hundred cases of coronary heart disease combined with diabetes mellitus who underwent lipid test in Zunyi Honghuagang District People's Hospital from January 2020 to March 2023 were selected as the study subjects.The patients were grouped according to the Gennisi score,which was low score group(n=40),medium score group(n=35),and high score group(n=25).The patients were tested for lipids,and the results of lipid tests and the ratio of each index were recorded and correlated with the Gennisi score.Results:The total cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)levels in the three groups were compared,and the differences were statistically significant(P<0.05);Comparison of TC/high-density lipoprotein cholesterol(HDL-C)and LDL-C/HDL-C in the three groups,the difference was statistically significant(P<0.05);After controlling for the influencing factors fasting glucose and HbA1c,the levels of TC/HDL-C and LDL-C/HDL-C were found to be positively correlated with the Gennisi score(P<0.001).Conclusion:In addition to conventional lipid indicators,TC/HDL-C and LDL-C/HDL-C levels also have high predictive value for the risk of coronary heart disease in diabetic patients,and can provide a reference basis for clinical diagnosis.
