摘要
目的:评估MEDUMAT Standard a型转运呼吸机在氧舱内拓展使用的可行性。方法:将呼吸机设置为间歇正压通气(intermittent positive pressure ventilation,IPPV)、通气频率(Freq)12次/min、纯氧(NO AIR MIX)模式,依据分钟通气量(minute volume,MV)不同分为a、b、c、d 4个实验组,在氧舱(0~120 kPa)加压、减压阶段记录可计量模拟肺显示的呼吸机实际输出Freq、潮气量(tide volume,TV),同时测试呼吸机报警性能。分析a、b、c组环境压力对TV的影响;修正d组MV调节旋钮值以保持模拟肺在不同环境压力下计量的TV(TVd)接近预设目标值(TV=500 mL),并编制《环境压力值-修正MV调节旋钮值对照表》。分析环境压力值与修正MV调节旋钮值的相关性,并比较TVd与预设目标值之间的差异。采用Excel 2016、SPSS 25.0软件对实验数据进行统计学分析。结果:氧舱环境压力变化对呼吸机Freq、报警系统性能无影响;环境压力值与呼吸机实际输出TV呈线性负相关(R2>0.95,P<0.001);依据《环境压力值-修正MV调节旋钮值对照表》修正MV调节旋钮值,可维持实际输出TV接近预设目标值。结论:在氧舱环境压力为0~120 kPa范围内,MEDUMAT Standard a型转运呼吸机可拓展为高压氧治疗机械通气设备。
Objective To evaluate the feasibility of expanding the use of MEDUMAT Standard a ventilator in the hyperbaric oxygen chamber.Methods There were four experimental groups(a,b,c and d group)formed according to minute volume(MV)with the ventilator set as the mode of intermittent positive pressure ventilation(IPPV),12 times/min ventilation frequency(Freq)and NO AIR MIX.The actual output Freq and tidal volume(TV)of the ventilator displayed by the measurable simulated lungs were recorded during the pressurization and depressurization phases of the oxygen chamber(0-120 kPa),and the ventilator alarm performance was tested at the same time.The effects of ambient pressure on TV were analyzed in groups a,b and c;the MV adjustment knob value in group d was corrected to keep the measured TV(TVd)of the simulated lungs at different ambient pressures close to the preset target value(TV=500 mL),and the"Comparison Table of Ambient Pressure Values and Corrected MV Adjustment Knob Values"was compiled.The correlation between ambient pressure and the MV adjustment knob values was analyzed,and the actual TVd values and preset target values were compared.The experimental data were statistically analyzed using Excel 2016 and SPSS 25.0 software.Results The changes in the ambient pressure of the oxygen chamber had no effect on the Freq values and alarm system performance of the ventilator;the ambient pressure values were linearly and negatively correlated with the actual output TV values of the ventilator(R2>0.95,P<0.001);regulating the MV adjustment knob values according to the"Comparison Table of Ambient Pressure Values and Corrected MV Adjustment Knob Values"contributed to keeping the actual output TV close to the preset target value.Conclusion MEDUMAT Standard a ventilator can be used for mechanical ventilation during hyperbaric oxygen therapy with 0 to 120 kPa ambient pressure in the hyperbaric oxygen chamber.
