角膜绷带镜在翼状胬肉手术患者中的应用效果

Application Effect of Corneal Bandage Scope in Patients with Pterygium Operation

目的 探究翼状胬肉切除(EOP)联合自体角膜缘干细胞移植术后配合角膜绷带镜(BCL)辅助治疗翼状胬肉患者的应用价值。方法 选取2021年2月至2022年8月我院80例(80眼)翼状胬肉患者,随机分成两组。对照组接受翼状胬肉切除(EOP)联合自体角膜缘干细胞移植(ALSCT)治疗,观察组在对照组基础上予以BCL辅助治疗。对比两组术前、术后7 d、14 d症状评分、角膜荧光素染色(CFS)试验、眼表疾病指数(OSDI)问卷评分、泪膜破裂时间(BUT)、角膜上皮恢复优良率、复发率。结果与对照组相比,术后7 d、14 d观察组各项症状评分更低,7 d、14 d观察组CFS、OSDI评分更高,BUT更长(P<0.05);与对照组角膜上皮恢复优良率80.00%相比,观察组角膜上皮恢复优良率为97.50%更高(P<0.05);两组复发率对比差异无统计学意义(P>0.05)。结论 在EOP联合ALSCT治疗翼状胬肉患者基础上,佩戴BCL辅助治疗能进一步促进角膜上皮恢复,缓解临床症状,减少复发,增强泪膜稳定性,降低对眼表环境的影响。

Objective To investigate the application value of pterygium excision(EOP)combined with limbal stem cell auto transplantation combined with corneal bandage scope(BCL)in the adjuvant treatment of patients with pterygium.Methods A total of 80 patients(80 eyes)with pterygium from February 2021 to August 2022 in our hospital were randomly divided into two groups.The control group received pterygium excision(EOP)combined with autologous limbal stem cell transplantation(ALSCT),and the observation group received BCL adjuvant therapy on the basis of the control group.Symptom score,corneal fluorescein staining(CFS)test,ocular surface disease index(OSDI)questionnaire score,tear film rupture time(BUT),good and good recovery rate of corneal epithelium and recurrence rate were compared between the two groups before and after surgery.Results Compared with the control group,the symptom scores were lower in the observation group at 7 days and 14 days after operation.CFS and OSDI scores were higher BUT longer in 7 days and 14 days observation groups(P<0.05).Compared with 80.00%in the control group,the recovery rate of corneal epithelium was 97.50%higher in the observation group(P<0.05).There was no significant difference in recurrence rate between the two groups(P>0.05).Conclusions On the basis of treating patients with pterygium with EOP combined with ALSCT,wearing BCL adjuvant therapy can further promote the recovery of corneal epithelium,relieve clinical symptoms,reduce recurrence,enhance the stability of tear film,and reduce the impact on ocular surface environment.
