摘要
目的此项研究旨在探索肝豆状核变性患者白质中弓状纤维束的损伤,及其与临床认知功能障碍的相关关系。方法采集就诊于安徽省中医院的肝豆状核变性患者24例和健康受试者26例,采用3.0T磁共振分别采集DTI序列,利用AFQ技术对大脑双侧弓状纤维束的100个节点进行自动定量分析。结果通过t检验及χ^(2)检验,性别和年龄在患者及健康受试者之间无统计学差异。经过AFQ分析,双侧弓状纤维各参数在患者与对照组之间存在统计学差异,表现为不同程度FA值降低,MD值、AD值和RD值升高。经过Pearson相关分析,发现左侧弓状纤维束FA值与MMSE及TBPM呈正相关(P<0.001),MD值及TBPM呈负相关(P<0.001);右侧纤维束FA值与MMSE及TBPM呈正相关(P<0.05),MD值与MMSE呈负相关(P<0.01),MD值及TBPM呈负相关(P<0.001)。结论参与学习及记忆功能的UF损伤可能在WD患者的认知功能障碍中发病机理中发挥重要的作用。
Objective To investigate the damage of arcuate fiber tracts in the white matter of patients with hepatolenticular degeneration and its relationship with clinical cognitive impairment.Methods24 patients with hepatolenticular degeneration and 26 healthy subjects who were treated in Anhui Provincial Hospital of Traditional Chinese Medicine were collected.DTI sequences were collected by 3.0T magnetic resonance imaging.Perform automated quantitative analysis.ResultsThere was no statistical difference in gender and age between patients and healthy subjects by t test andχ^(2)test.After AFQ analysis,there were statistical differences in the parameters of bilateral arcuate fibers between the patients and the control group.After Pearson correlation analysis,it was found that the FA value of the left arcuate fiber bundle was positively correlated with MMsE and TBPM(P<0.001),and the MD value and TBPM were negatively correlated(P<0.001);the FA value of the right fiber bundle was positively correlated with MMsE and TBPM.Correlation(P<0.05),MD value was negatively correlated with MMsE(P<0.01),MD value and TBPM were negatively correlated(P<0.001).ConclusionThe UF damage involved in learning and memory function may play an important role in the pathogenesis of cognitive dysfunction in WD patients.
